About this file

This is a Response Evaluation Scorecard template from the Division of Marine Fisheries, with Tracy Pugh listed as the Procurement Team Leader and T. Pugh, D. Perry, and J.Swinford as evaluators. The document outlines the evaluation criteria and scoring methodology for what appears to be a marine vessel procurement or charter services contract. The scorecard uses a 0-5 point scale ranging from "No Score" (0 points) to "Excellent" (5 points) to evaluate key criteria including bid price, boat size/specs/speed, captain's experience, estimated steam time, and past performance.

The evaluation process requires the Procurement Team Leader to assign multipliers to each criterion before distribution to evaluators, with the total maximum points available required to equal 100. The form is designed to be used as a fillable PDF, with the first three items permanently set and the last three items configurable for different evaluation criteria. The document includes a section for zone awarded, suggesting this may be part of a zoned or regional contract award structure. No specific pricing terms, funding requirements, or contract values are included in this template document.

USING THIS FORM:This Evaluation Scorecard is best used as a fillable PDF. Only the first 3 items are permanently set, the last 3 items (AMP, SOWMBA, Past Performance) may be tabbed to removed or typed over if the PMT is using different evaluation criteria. The PMT must fill-in a multiplier prior to distributing to evaluators. After filling in the 'Multiplier' column, make sure the Total for 'Maximum Points Available' = 100.(The entries below are examples that should be changed by the PMT).
