Description/Specifications/Statement of Work
STATEMENT OF WORK (SOW)
CLINICAL-GRADE DRY ICE PELLET DELIVERY SERVICES
WOMACK ARMY MEDICAL CENTER (WAMC), FORT BRAGG,NC
1.0 SCOPE OF WORK
The Contractor shall provide all labor, transportation, packaging, equipment, and materials necessary to manufacture, transport, and deliver bulk clinical-grade dry ice (5/8" pellets) twice weekly to two designated clinical locations at Womack Army Medical Center (WAMC), Fort Bragg, NC.
2.0 ESSENTIAL CHARACTERISTICS
Physical Specifications: The dry ice must be delivered in strictly 5/8" diameter pellet form.
Clinical Specimen Preservation Rationale: Pellets smaller than 5/8" in diameter (such as 1/2" or smaller) sublime at an accelerated rate, which fails to maintain the strict thermal stability necessary for pathology and blood specimen preservation. Smaller pellet sizes are also physically incompatible with WAMC's clinical specimen transport canisters. Any delivery of dry ice pellets measuring less than 5/8" in diameter shall be rejected in its entirety.
Quality Standard: The product must be free from contaminants and suitable for a clinical laboratory and blood bank environment.
3.0 LOGISTICAL DELIVERY REQUIREMENTS
Deliveries shall be made twice weekly, every Monday and Thursday, to the following two clinical locations. If a Federal holiday falls on a scheduled delivery day, delivery shall be made on the next standard duty day.
3.1 Department of Pathology (CLIN 0001 Series)
Delivery Location: Shipping Department, Ground Floor, Building 4-2817 Rock Merritt Ave, Fort Bragg, NC 28310.
Quantity per Delivery: 150 Pounds (300 Pounds weekly).
Delivery Cut-Off Time: No later than 11:00 AM EST.
3.2 Blood Donor Center (CLIN 0002 Series)
Delivery Location: Building-4156, Souter Street, Fort Bragg, NC 28310.
Quantity per Delivery: 250 Pounds (500 Pounds weekly).
Delivery Cut-Off Time: No later than 3:30 PM EST.
3.3 Annual Quantities Summary:
Standard Years (Base, OY2, OY3, OY4): 15,600 Lbs for Pathology; 26,000 Lbs for Blood Donor Center (Total: 41,600 Lbs/year).
Option Year 1 (53-Week Calendar Year): 15,900 Lbs for Pathology; 26,500 Lbs for Blood Donor Center (Total: 42,400 Lbs).
4.0 INSPECTION AND ACCEPTANCE (PERFORMANCE STANDARDS)
Destination Inspection: The Contracting Officer's Representative (COR) will inspect each shipment upon arrival for quantity, pellet diameter (5/8" minimum), and signs of excessive sublimation (melting/shrinkage).
Deficiency Remedy: The Contractor shall replace any rejected, non-conforming, or degraded shipment within four (4) hours of verbal or written notification from the COR or Contracting Officer at no additional cost to the Government.
5.0 BASE ACCESS AND SECURITY REQUIREMENTS (NON-CAC HOLDERS)
(a) Background Checks: All Contractor delivery personnel who do not possess a Common Access Card (CAC) must comply with local gate-access procedures.
Personnel shall undergo background checks utilizing the National Crime Information Center Interstate Identification Index (NCIC-III) and the Terrorist Screening Database (TSDB) prior to being granted entry.
(b) Gate Clearance: Drivers must present a valid, unexpired State-issued driver's license, active commercial vehicle registration, and active vehicle insurance at the designated visitor control center or truck commercial inspection gate.
(c) Force Protection Changes: Should the Force Protection Condition (FPCON) at Fort Bragg escalate, the Government may restrict base access. The Contractor is responsible for coordinating with the COR to adjust delivery schedules as authorized by the Contracting Officer.
Requirements A bi-weekly delivery of dry ice to the Department of Pathology and the Blood Donor Center.

Evaluation Factors for Award
ADDENDUM-1 TO: 52.212-2 - EVALUATION CRITERIA
(a) Evaluation Factors for Award The Government will award a contract resulting from this Request for Quotation (RFQ) to the responsible offeror whose offer conforming to the RFQ represents the best value to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
Factor 1 - Technical Capability Factor 2 - Price
(b) Basis for Award Award of this contract will be made on a competitive best value basis, using Lowest Price Technically Acceptable (LPTA) source selection procedures as described in FAR 15.103-2 (RFO version), in accordance with FAR 12.201-1(d).
Award will be made on the basis of the lowest evaluated price of the quotation meeting or exceeding the requirements for acceptability. Evaluation will be made of the offeror whose proposal is technically acceptable in accordance with this RFQ, and the evaluation criteria set forth below. The offeror's proposal will be evaluated to ensure that the Government receives a technically acceptable proposal at a fair and reasonable price. Technical acceptability will be determined by evaluating the following non-price factor: Factor 1 - Technical Capability.
Government evaluators will not assume that the offeror possesses any capability or knowledge unless it is explicitly specified in the offeror's quote. The non-price Factor 1 will be rated individually and separately from price Factor 2. If non-price Factor 1 receives an individual rating of "Unacceptable," the collective overall rating will also be "Unacceptable." Only proposals receiving an overall rating of "Acceptable" will be considered for award.
(c) Evaluation Criteria Offers for this requirement will be evaluated using the following criteria:
Factor 1 - Technical Capability:
The offeror's proposal for this requirement will be evaluated using the criteria shown in the table below. An evaluation of technical acceptability will be made to ensure satisfactory responsiveness to the requirements set forth in this RFQ. The non-price Factor 1 will be rated as either "Acceptable" or "Unacceptable".
Rating Description Acceptable Quotation meets the requirements of the RFQ.
Unacceptable Proposal does not meet the requirements of the RFQ.
The Government will evaluate the degree to which the offeror's technical quotation demonstrates that the offeror can provide the requested dry ice 5/8" pellets within the parameters described in the RFQ and can meet the strict twice-weekly clinical delivery schedule. The vendor shall clearly list out if they meet or do not meet requirements listed in the RFQ. If the offeror cannot meet a specific Essential Characteristic (EC) as defined in the purchase description, the offeror shall explain why they do not meet that requirement.
To receive an overall rating of "Acceptable," the offeror shall provide a written quotation that clearly reflects the technical capability to provide the required supplies within the requested timeframes, and their understanding of the requirements set forth in the EC. Quotations shall be clear, coherent, and prepared in sufficient detail for effective evaluation of the offeror's proposal against the evaluation criteria. The Government will evaluate only one (1) quotation from the prime offeror and the maximum page limit for Technical Capability is three (3) pages. The Government will not accept alternative quotations.
Factor 2 - Price:
The offeror shall include a total price for each CLIN identified in the RFQ and shall include shipping cost in the unit price (FOB Destination). The offeror's proposed price shall be evaluated separately from the non-price evaluation factor. Price evaluation is to determine whether the offeror's proposed price is fair and reasonable in accordance with FAR 12.204 (RFO version). Price analysis will be used to determine the total evaluated price to support the selection of the best value offered to the Government.
(d) Options The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options shall not obligate the Government to exercise the option(s).
(e) Notice of Award A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror(s) within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
