About this file
This is a Past Performance Questionnaire Form designed to evaluate contractor performance on previous contracts. The form is structured to collect detailed feedback from government evaluators on a contractor's historical performance across five primary evaluation categories: Quality of Product or Service, Timeliness of Performance, Cost Control, Customer Satisfaction, and Business Relations. Each category uses a five-level rating scale ranging from Exceptional to Unsatisfactory, with specific descriptive definitions provided for each level.
The questionnaire requires evaluators to complete sections identifying the contractor being assessed, contract details (including contract number, award date, completion date, total contract amount, and contract type), and the evaluator's agency and contact information. Evaluators must provide ratings and detailed comments for each performance element, answer yes/no questions regarding contract compliance issues (such as quality deficiencies, late deliveries, terminations, litigation, and quality awards), and provide a summary opinion selecting from five predetermined statements describing their overall experience with the firm. The form concludes with an evaluator certification section requiring signature, printed name, title, and date. This template is used as part of the solicitation process for the Type 6 Wildland Light Engine Upgrade for Kirtland (Solicitation 140D0426Q0416) to assess the past performance of prospective contractors before award.
