REQUEST FOR PROPOSAL

(RFP)

Proposal Submission Package

NATIONAL PARK SERVICE

DENVER SERVICE CENTER

For Technical Volume 1

1. FORMAT: Submit (1) one electronic copy. No hard copies shall be submitted.

2. ORGANIZATION: Include folders and files for electronic formats named as follows:

3. CONTENTS:

FACTOR 1 – TECHNICAL APPROACH

Technical approach is your means and methods to complete a technical aspect of required work in this solicitation. Submit a written narrative limited to 10 pages describing your technical approach to the specific aspects below. Additional pages will not be evaluated. Pictures can be included and will count towards the page limitation. There is no preprinted form for this submittal.

· Approach to Critical Contract Requirements: Describe your approach to successfully accomplish demolish existing asphalt, install drainage, concrete paving, and phasing and sequencing in accordance with requirements contained in the specifications and drawings within proposed contract performance period and budget. Identify potential related risks or reasons for project time or cost impacts and your solutions to mitigate and resolve them.

· Risk Analysis: Submit a risk analysis identifying critical issues, potential problems, and/or areas of concern with recommended solutions. Include strategies to manage project risks such as weather and working around unforeseen site conditions, material lead times, and work force availability.

· Natural/Cultural/Historic Protection & Preservation Plan: Describe your approach to protection and preservation of natural/cultural/historic resources as required in the solicitation, including but not limited to requirements in Specification Section 01 11 00-1.

FACTOR 2 – PROJECT EXPERIENCE

Submit the attached "Project Experience" form addressing specific experience aspects below for up to 3 relevant projects completed in the last 5 years. Submit separate forms for each project. NOTE: Submit the same projects for Past Performance (Factor #7) as submitted under this factor.

Project experience demonstrating:

(1) Capability to successfully complete relevant projects of similar scope and magnitude to this solicitation.

(2) Strong technical knowledge and skill required for this solicitation; and

(3) Effective management, problem solving, communication, coordination, and organization present lower levels of risk and will be more favorably rated. Those not demonstrating these attributes present higher levels of risk and will be less favorably rated.

FACTOR 3 – SCHEDULE

Submit your proposed construction schedule for THIS SPECIFIC PROJECT showing completion within the required period of performance contained in this solicitation with accompanying schedule narrative. Submit your schedule in MS Project Gantt Chart format; for evaluation purposes only assume a Notice to Proceed date of October 15, 2022; and include specific activities required for this project and a clear critical path with major milestones and applicable predecessor activities. In your accompanying narrative, explain schedule logic, critical path activities, and key considerations and constraints. There is no preprinted form for this submittal.

FACTOR 4 – MANAGEMENT APPROACH

Management Approach is organizational structure and business processes to manage, control, and oversee your firm's operations. Submit a written narrative limited to 5 pages describing your management approach to the specific aspects below. Additional pages will not be evaluated. Pictures can be included and will count towards the page limitation. There is no preprinted form for this submittal.

Management approaches demonstrating:

(1) Sound management practices and business processes.

(2) Effective cost and quality control; and

(3) Clear corporate and project team commitment, hierarchy and lines of communication, roles, responsibilities, and relationships present lower levels of risk and will be more favorably rated. Plans not demonstrating these attributes present higher levels of risk and will be less favorably rated.

FACTOR 5 – KEY PERSONNEL EXPERIENCE

Submit resumes for key personnel as identified in Section H of this solicitation and listed below. All personnel proposed are subject to substitution requirements contained in this solicitation. There is no preprinted form for this submittal.

· Project Manager

· Superintendent

· On-site Quality Control Specialist

· On-site Safety Specialist

· Professional Engineer

· Testing Agency

FACTOR 6 – SAFETY

Submit an overview of your company’s Construction Safety Program and an assessment of the safety risks associated with this construction project. Discuss any changes to your company’s Construction Safety Program required to address your assessment of the project risk. Include the names, resumes, and specific safety training completed (i.e., OSHA, etc.) for the proposed Project Site Safety Manager for this project. Additionally, submit a copy of OSHA 300A logs for the previous three calendar years along with a statement from your insurance company showing your company’s current Experience Modification Rate (EMR).

FACTOR 7 – PAST PERFORMANCE

Submit Past Performance information for the same projects submitted under Project Experience (factor 7) using the attached Past Performance Questionnaire. Information may be regarding the offeror's firm and/or its proposed subcontractors performing key trades and other construction activities. Subcontractor past performance will only be considered if it correlates to the work the subcontractor will be performing on this requirement. If available, offerors may submit a Past Performance Information Retrieval System (PPIRS) evaluation report in lieu of the Past Performance Questionnaire. Submittal process for the Past Performance Questionnaire is as follows:

1) Complete Sections I and II only of the attached Past Performance Questionnaire and forward to your client reference. The reference shall be someone who was substantially involved with the project and can provide a complete, objective assessment of your performance. Direct them to complete and submit the survey to the address and point of contact in paragraph B above by the due date and time for proposal receipt.

2) Submit copies of the questionnaire that you forwarded to your references with their point of contact information in this proposal Volume I.

3) Submit a narrative disclosing any instances of past performance that may be considered less than fully satisfactory. Describe pertinent facts, circumstances and any remedial action taken or to be taken to correct the deficiency. Failure to disclose such instances could result in an unfavorable assessment of the offeror's past performance record.

4. Subcontractor Reference Form

Include the following submittals not related to any evaluation factors in Volume I.

For Business & Price Volume II:

1.FORMAT: Submit (1) one electronic copy. No hard copies shall be submitted.

3. CONTENTS:

SF1442: Standard Form 1442, Solicitation, Offer, and Award, (Construction, Alteration, or Repair): Complete blocks 14 through 20C only. An official having the authority to contractually bind the offeror's company must sign the SF 1442 in accordance with the procedures prescribed in FAR 4.102.

AMENDMENTS: DO NOT acknowledgement amendments through FedConnect. Acknowledge all solicitation amendments using one of the methods set forth in Block 11 of Standard Form 30.

SECTION B CONTRACT PRICE SCHEDULE: Enter the proposed dollar amount for each line item in the Price Schedule, including any optional items, if applicable. Also provide a separate Schedule of Values (SOV) for each line item in the Section B Contract Price Schedule. Each SOV shall show the line item price broken down by major work components.

SF 24 BID BOND: Submit Standard Form 24, Bid Bond duly executed between the offeror and the bonding company. Refer to clause 52.228-1.

SECTION K REPRESENTATIONS AND CERTIFICATIONS: Submit a completed copy of provision 52.204-8 shown in section “K” of this solicitation. Complete all required representations and certifications if they are not available at www.SAM.gov. Your firm must maintain current, active registration at www.SAM.gov to be eligible for award and you are encouraged to complete SAM registration before submitting your offer.

GENERAL REFERENCES: Complete and submit the attached "General References," form. Information will be used in determining Responsibility pursuant to FAR 9.103.

SUBCONTRACTING PERCENTAGE WORKSHEET: Complete and submit the attached "Report on Limitation on Subcontracting" form. The form itself requests input of amounts paid; however, for solicitation purposes, input projected amounts based on your proposed pricing and that of your anticipated subcontractors. Refer to Clause 52.219-27 for associated information.

The NPS subcontracting goals for Fiscal Year 2022 are: Small Businesses (44%), Small Disadvantaged Businesses (5%), Women-Owned Businesses (5%), HUBZone (3%), and Service Disabled Veteran-Owned Businesses (3%).

Subcontracting plans that contain goals less than the NPS goals shall include rationale as to why the above goals cannot be met.
