Statement of Work
I. Project Identification
A. Title: Demolition and Removal of Reinforced Concrete Stilling Well, Headwall, and Associated Structures

II. Objectives
To demolish and remove a discontinued data collection station primarily consisting of a 6 ft. diameter, 25 ft. tall, 8 in thick, poured concrete stilling well with a 6 in. thick,10 ft x 10 ft concrete slab apron at the previous ground level and extending ~5 ft. below apron. Additional gage structures for demolition and removal include a 11 ft x 21 ft x 4 ft concrete headwall. An additional section of headwall ~7 ft. x 20 ft. x 1 ft.; as well as any associated debris that has fallen into the main channel of the Yadkin River and destabilized the banks. After removal, any holes will be filled with an appropriate material to prevent entry, and the bank will be stabilized and restored to ‘natural’ conditions. The awarded contractor will be removing the structure and hauling away all debris including the attached metal railing, ladders, pipe, conduit, wire, and platform.
III. Special Requirements
The site is located is on private property. Contractor will be responsible for coordinating access with the landowner. Contractor will limit disturbance to the property and return any disturbed areas to conditions sufficient to the landowner. Land between Trails End Lane and the demolition site is flat, but the ground is soft. This area must be repaired to conditions similar or better than they were found.
Work under this contract shall be performed in accordance with the requirements of this Statement of Work, specifications for this contract, and all applicable federal, state and local regulations. It is very important that the successful bidder understand that this project includes removal of debris made by deconstruction.
IV. Inspections, certifications and compliance
A. Contractor shall provide complete drawings for approval prior to construction.
B. Contractor shall obtain all applicable municipal and transportation permits.
C. Contractor will be responsible for ensuring that all debris will be removed from the site.
D. Contractor will be responsible for returning area to as close to natural as possible, while also stabilizing the bank in a way that limits future erosion.

VI. DELIVERIES OR PERFORMANCE
Commencement, Prosecution, and Completion of Work: Normal working hours shall be 7:00am-5:00pm, Monday –Friday. The contractor shall not work Saturdays, Sundays, legal holidays, or non-normal working hours, unless otherwise authorized by the Contracting Officer’s Technical Representative (COTR). Request to work times other than normal hours shall be made in writing. The contractor shall employ sufficient personnel and equipment and order supplies to accomplish the work of this contract within 90 calendar days of the Notice to Proceed.
Progress Schedule: The Contractor shall submit a project schedule to the Contracting Officer’s Representative (COR) for approval within five days after receipt of the Notice to Proceed. The progress schedule shall be a chart, which graphically depicts the proposed sequence to accomplish each work feature or operation. It shall show start dates and completion dates for each event on a horizontal time scale. The progress schedule time scale shall begin with the Notice to Proceed and indicate the number of days to completion. The schedule will be the reference to which the contractor progress and effort are compared.
SPECIAL CONTRACT REQUIREMENTS
List of Subcontractors: For purposes of monitoring compliance with the insurance and payroll reporting requirements of this contract, the contractor shall furnish a list of all subcontractors to the Contracting Officer. This list shall be sent with the performance and payment bonds.
Transporting of Debris and Material: All materials and construction debris shall be removed from the work area and removed from the site daily.
Disposal of Debris: All materials and debris shall be disposed of properly by the contractor.
Organization at the site: Contractor will coordinate site access with the landowner and establish protocols based on landowner preference. Contractor will limit disturbance to the property and restore property to pre-construction conditions or better.
Site Cleanliness: The contractor shall maintain the site free of construction debris in contractor furnished containers (barrels, bags, dumpsters). The contractor maintains cleanliness of the work site by promptly removing debris, tools, and excess material.
Contractor Parking and Traffic Control: Contractor parking is limited along the shoulder of Trails End Lane with additional parking at Happy Valley Park to the south across NC Highway 268. Contractor is responsible for all aspects of Traffic control required to complete this project in accordance with NCDOT requirements.
Environmental Protection: The contractor shall perform all work and activities in accordance with local, state, and federal environmental regulations.
Hot Work: Contractor will always supply fire extinguishing equipment onsite to prevent fires resulting from any operations including but not limited to torch cutting or grinding.
Personal Protective Equipment (PPE): PPE to be provided by the contractor or Subcontractor to all employees and always used on the site, including, but not limited to Safety toe boots or shoes, Safety glasses, Hard hats, High visibility vests, and fall protection.
First Aid and Rescue Equipment: Contractor must have first aid kit sufficient for number of workers onsite.
Outages and Interruptions: USGS streamgage data collection is on-going in this location. Cooperator will work closely with designated USGS Asheville Field Office staff to coordinate construction activities in a manner that limits disruption to on-going data collection activities. This will include coordinating areas to not disturb where current streamgage equipment and infrastructure are located. Additionally, the contractor is expected to coordinate the timing of their activities closely with Asheville Field Office staff so they can appropriately screen data from the web and coordinate site visits when necessary to maintain data integrity throughout construction.
Notice to the Government of Delays: In the event the contractor encounters difficulty in meeting performance requirements, or when it anticipates difficulty in complying with the contract delivery schedule or date, or whenever the contractor has knowledge that any actual or potential situation is delaying or threatens to delay the timely performance of this contract, the contractor shall immediately notify the Contracting Officer, in writing, giving pertinent details. This data shall be informational only in character. Notice under this provision shall not be construed as a waiver by the Government of any delivery schedule or date or any rights or remedies provided by law or under this contract.
Pre-Construction Meeting: The Contractor shall have the project manager available on site for a pre-construction meeting prior to the commencement of work for this project. The COTR will conduct a pre-construction meeting to review the requirements and conditions of this contract.

All work listed shall be coordinated with the COTR and /or the Asheville Field Office Chief.
