About this file

This document is a "FACTS Sheet" template for offerors to provide past performance references on three relevant contracts for construction services similar in scope and size to the current federal acquisition.

The template requests information on three past projects, including contract details, descriptions of work, dollar values, delivery timeliness, and references. At least one project must involve work on a structure listed in the National Register of Historic Places. For newly formed entities, key personnel from previous companies who will support the proposed effort must be identified. The document concludes by noting the government will perform an independent relevancy assessment of the provided data.
