About this file

This is a Past Performance Questionnaire (PPQ) template used by the U.S. Department of the Interior, National Park Service for evaluating contractor performance on the GATE – Repair Ft. Tompkins Earthen Roof Structure project (Contract Number 140P4526R0010).

The questionnaire is a two-section document designed to be completed collaboratively between the offeror and project owner/evaluator. Section A requires the offeror to provide project details including project title, location, description, company name, Unique Entity ID/CAGE Code, owner/project manager contact information, start and completion dates, and initial and final contract amounts. Section B requires the evaluator (project owner or reference) to assess contractor performance across nine rating criteria using a five-point scale: Outstanding (O), Above Average (A), Satisfactory (S), Marginal (M), or Unacceptable (U). The evaluation addresses relationship management, corporate integrity, quality control, schedule performance, any enforcement actions taken, customer satisfaction, and overall rating. Offerors may submit between 3-5 completed questionnaires with their proposal, and past performance projects must have been completed within five years of contract completion. The completed questionnaire must be received by the contracting point of contact by the deadline specified in the solicitation; late submissions will not be accepted. Electronic submissions should account for potential transmission delays.
