About this file

This is an Indian Affairs Indian Economic Enterprise Representation Form used for federal contract solicitations under the Buy Indian Act.

The form is a self-certification document for offerors responding to solicitation 140A2326Q0205 for "Wardrobe Closets, SIPI." By submitting the form, the offeror certifies that it meets the definition of an "Indian Economic Enterprise" (IEE) as defined in the Department of the Interior Acquisition Regulation (DIAR) Part 1480. The enterprise must maintain IEE status at three critical points: when submitting an offer in response to the written solicitation, at the time of contract award, and throughout the full contract term. Contracting Officers may request additional eligibility documentation at any point during the acquisition lifecycle. The form requires offerors to provide the federally recognized tribal entity name, unique entity identifier (UEI), legal business name, name of the 51% or greater owner of the IEE firm, a certifying signature, and the printed name of the signatory. False or misleading information submitted during the offer process violates 18 U.S.C. 1001, and false claims during contract performance are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
