About this file
The document is a Past Performance Questionnaire for Request for Quote (RFQ) #140P5125R0006, designed to capture a contractor's prior experience in providing landscaping and groundskeeping services. The form requires offerors to list up to three completed jobs within the past five years, including details such as project name, location, owner information, project value, dates of service, and a description of the project and scope of work. The questionnaire emphasizes the importance of transparency, encouraging offerors to disclose any instances of less-than-fully-satisfactory performance and explain corrective actions taken or planned.
The associated federal contract opportunity is for landscaping services at Fort Moultrie National Historic Site, the United States Coast Guard Life Saving Station on Sullivan's Island, and Charles Pinkney National Historic Site in South Carolina. The National Park Service plans to award a single Firm-Fixed Price Contract using a Lowest Priced Technically Acceptable (LPTA) source selection procedure. The contract is classified under NAICS code 561730 with a small business size standard of $9,500,000, and the estimated performance period is twelve months with potential option years. Offerors must be registered in SAM.gov, self-certified under the specified NAICS code, and are highly encouraged to participate in a site visit before submitting a proposal.

Past Performance Questionnaire READ CAREFULLY: The purpose of this form is for the offeror to list services performed during the past five (5) years that are similar to the work described in this solicitation. The number of projects listed shall not exceed five (5) and shall be numbered consecutively, beginning with the most recent project. Use this form to address your familiarity with the Statement of Work. Include Area Codes with all telephone numbers. Fill in the information by typing or printing legibly.
It is especially important that the offeror disclose instances in which their past performance may be considered by others to be less than fully satisfactory. The offeror should tell their side of the story and describe remedial corrective action that has been taken or will be taken to correct the deficiency. Failure to do so may result in a determination that an offeror has been less than candid with the Government, which could result in an unfavorable assessment of the offeror’s past performance record.
1. Please explain in detail your company's prior experience in providing Landscaping/groundskeeping Services similar to this requirement:
