About this file
This is a Past Performance Questionnaire template for RFP 140P5126Q0001 issued by the National Park Service, Contracting Operations (ConOps) East. The questionnaire requests references to complete evaluations of contractor performance for a proposal to repair the Control Tower Observation Deck and replace the fire escape system at Tuskegee Airmen National Historic Site in Tuskegee, Alabama. The project requires a Historical Preservation team to perform these services per the statement of work (SOW).
The questionnaire must be completed and submitted with the quote prior to the closing of RFP #140P5126Q0001. References are requested to provide information on the contractor's current or previous contract performance, including contract number, value, period of performance, and description of items/services provided. The evaluation section uses a five-tier rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or N/A) to assess contractor performance across five areas: overall ability to meet requirements, timeliness of product delivery, ability to resolve timeliness problems, quality of work/shipments, and overall contractor performance regarding requirements, schedule, and price. Respondents are instructed to provide detailed explanatory narratives for ratings. The questionnaire includes a notice to contractor stating that responses are source selection sensitive and cannot be used for promotional or endorsement purposes.
