About this file

This is a Past Performance Questionnaire (PPQ) template for solicitation W519TC-25-R-0014 for the demilitarization of shotgun cartridges, issued by Army Contracting Command - Rock Island (ACC-RI).

The PPQ requires offerors to complete sections IIA and IIB with their past performance references and submit them with their proposal, while the PPQ respondents must complete all other sections and submit directly to the contract specialist and contracting officer. The questionnaire evaluates contractors on a 5-point scale (Excellent to Unsatisfactory) across multiple performance areas including: meeting contract quality and demilitarization requirements, ability to meet schedules, problem identification and correction capabilities, overall work satisfaction, and compliance with federal/state regulations. The form also collects detailed contract information such as total demilitarization quantities, monthly rates, contract values, and places of performance. Any Corrective Action Reports (CARs) or contract terminations must be disclosed and explained. Completed questionnaires must be submitted to the designated ACC-RI contract specialist and contracting officer via email.
