About this file

This is a Past Performance Questionnaire template designed for evaluating contractor performance on federal projects. The document is structured for completion by both the contractor and the reference/client, with sections for contractor information, contract details, reference contact information, and comprehensive performance evaluations. The questionnaire establishes a five-tier rating scale (Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory) across five key evaluation areas: Quality, Schedule & Time Management, Cost Control, Management, and Small Business Subcontracting & Labor Standards, with additional sections addressing Regulatory Compliance.

The evaluation framework requires detailed assessments of specific performance metrics within each category. Quality evaluations address workmanship, quality control adherence, and technical submittals. Schedule evaluations measure milestone achievement, responsiveness to requests, and timely issue resolution. Cost Control assessments examine handling of site conditions, modification pricing fairness, invoice accuracy, and subcontractor payment promptness. Management ratings evaluate project oversight, subcontractor coordination, safety performance, location factor management, owner relationships, and problem-solving capabilities. Small Business Subcontracting and Regulatory Compliance sections include yes/no determinations regarding labor standards, disadvantaged business contracting, safety code compliance, and overall recommendations for future contract suitability. Completed questionnaires must be returned to Molly Lynam, Contract Specialist at the National Park Service Northern Rockies MABO, and are designated as Source Selection Sensitive information intended solely for evaluating contractor capability on solicitation 140P1426R0010.
