ATTACHMENT C - SMALL BUSINESS COMMITMENT DOCUMENT

Other than small businesses (SB) Offerors are required to complete a Small Business Commitment Document (SBCD). The Offeror shall propose the level of commitment of SB concerns (e.g., Small Disadvantaged Business (SDB) (including ANCs and Indian Tribes), Woman-Owned Small Business (WOSB), Historically Underutilized Business Zone (HUBZone), Veteran-Owned Small Business (VOSB), or Service-Disabled Veteran-Owned Small Business (SDVOSB)) in the performance of the acquisition relative to the objectives/goals set-forth in the evaluation of this factor.

1. Indicate the total contract dollar value and percentage of work based on total contract dollars of the acquisition to be performed by both large and SB (include the percentage of work to be performed by both the prime and subcontractors):

For example: If the Prime proposes a price of $1,000,000 (including all options), and SB will provide $250,000 in supplies/services as a prime or subcontractor, the %-age planed for SB is 25%; and 75% for large business equating to 100%.

2. Indicate the total combined dollar value and percentage of work based on total contract dollars of the acquisition to be performed by each SB concerns. The percentage of work performed by SB that qualify in multiple SB concerns should be counted in each status.

For example: Subcontractor A (a VOSB, SDVOSB, and WOSB) performing 2%; and Subcontractor B (a SDB, HUBZone, and WOSB) performing 3%. The results will equate to SB 5%, SDB 3%, HUBZone 3%, WOSB 5%, SDVOSB 2%, and VOSB 2%).

3. Identify the Prime Offeror and type of supply/service that the Prime will provide. Then list each of the intended SB concern subcontract and principal supply/services to be provided by the SB concern. Provide the Dun and Bradstreet Number (DUNS) and/or Commercial and Government Entity (CAGE) code for the Prime and each intended subcontractor. Also, provide the anticipated North American Industry Classification System (NAICS) code(s) that the Prime believes best describes the supply/service being acquired by the subcontracts with each intended subcontractor. SB subcontractors that qualify in multiple SB concern(s), as previously mentioned, should be listed in each applicable small business category.

Example: If a SB concern qualifies as a WOSB and a SDVOSB, the Offeror can add them to each category below in which they qualify.

Note: Pursuant to the Small Business Act, Section 8(d), a business is considered small for Government procurement if it does not exceed the size standard for the NAICS code that the prime contractor believes best describes supply/service being acquired. In other words, the size of the prime’s supplier is determined by the applicable NAICS code of their joint-venture, teaming, partnership, or subcontract, which may or may not be the same NAICS code as the one for the prime contract with the Government.

4. Describe the extent of commitment to use small business concerns (e.g., what types of commitments and of what duration, if any, are in place for this specific acquisition either – small business prime, written contract, verbal, enforceable, non-enforceable, joint-venturing, mentor-protégé, etc.).

5. When the type of commitment noted is a written commitment, provide copies of written commitment agreements. Copies of such agreement will not count against the proposal page limitation.
