About this file
This is a Questions and Answers amendment document issued by the State of Illinois Department of Financial and Professional Regulation (DFS) for Invitation for Bid #27-493ISP-FOREN-B-52569 regarding DNA Buccal Swab Kits. The document addresses six key clarifications submitted by potential bidders concerning the specifications and dimensions of the swab kit components. The primary items under procurement include custom-sized swab envelopes with maximum dimensions of 8 inches length by 3 inches width, return envelopes with specifications ranging from 9.5 inches length by 5 inches width (minimum) to 11.25 inches by 6 inches (maximum), anti-moisture desiccant packets, perforated edges, biohazard seals, barcodes, and shipping seals. The amendment clarifies that the specifications referenced in Attachment A (Sample Forms Specs_Revised_09172026) are integral to the contract requirements, with detailed visual mockups and photographs provided on designated pages for bidder reference.
The document establishes that specifications are non-negotiable, with the State of Illinois declining to accept alternative envelope dimensions that would deviate from the tri-folded configuration required for sample submission and sterile swab storage. Key clarifications include confirmation that desiccant packets function as hygroscopic drying agents to maintain low-moisture environments, that address information on return envelopes shall be printed in red ink for hazardous material identification, and that the "Affix Shipping Seal Here" marking appears on the return envelope flap. The amendments emphasize strict adherence to dimensional and material specifications outlined in the contract document Section 1.2.A references, with no deviations permitted to accommodate cost reduction or alternative manufacturing practices.

| Question |
Regarding the Swab Envelope: The maximum of 8” length x 3 ¼” width specification is a custom size and raises the cost. Would you consider a 3 5/8 by 61/2 envelope instead?
Answer No. The 3 5/8 would be too wide for the tri folded state of the sample for submission. (see Sample Forms – Attachment A document for Specification visual). The 6 ½ envelope length would be too short to house the required sterile swabs for submission.

| Question |
The swab envelope requires a desiccant packet equivalent to Desiccant. Can you provide a description for Desiccant please?
Answer
Desiccant is a hygroscopic substance used as a drying agent; it absorbs moisture to sustain dryness.
A desiccant packet is a small pouch filled with substances that create a low-moisture environment to protect packaged items from damage from moisture.
Section 1.2.A.3 of the IFB Contract Document wording updated for clarity: The envelope must contain an anti-moisture desiccant packet. There is also a photo example of the Desiccant pack on Page 5 of the Attachment A Sample Form Specs_Revised 09172026.
Question The return envelope description in the contract and the images in attachment A are different.
Contract says min size of 9.5”L to the fold x 5”w and max size 11.25”X6” – hat fold? I do not see one Attachment A says max size 8” long. Which is it?
Answer The Attachment A reference stating max size 8” long is for the envelope that will contain the swab. The return envelope is for returning the entire swab kit once processed. The swab envelope is shown on page 8 of Attachment A and addressed in Section 1.2.A.3
The return envelope is address in Section 1.2.A.1 of the contract. A Mock up of the return envelope and an actual photograph of the return envelope can be found on Page 9 (mock up) and page 11 (actual photo) of Attachment A – Sample Forms Specs_Revised 09172026.
The swab envelope (Max 8” in length) is addressed in Section 1.2.A.3 of the contract.
Question Attachment A says there is a perforated edge. That is not in the contract
Answer:
The perforated edge and it’s placement is addressed in Section 1.2.A.2 of the contract. A visual of the perforation can be found on page 2 of the Attachment A – Sample Forms Specs_Revised_09172026 document
The perforation specifications are not related to the return envelope.
Question:
“Affix Shipping Seal Here” is on the edge of the flap, but with the swab envelope name, biohazard seal and barcode, where would it go?
Answer:
The “Affix Shipping Seal Here” will go on the flap of the return envelope. See Page 11 of the Attachment A- Sample Forms Specs_Revised_09172026 document
6.
Question:
Why would the address information be printed in red ink? Attachment A shows it in black ink.
Answer:
Page 10 of Attachment A – Sample Forms Specs_Revised_09172026 is a mock up. An actual photograph of the return envelope has been added to this document on Page 11.
Red ink gives an immediate visual identification of hazardous or regulated contents.
