About this file

This Scope of Work document is issued by the State of Illinois Department of Human Services for the Illinois School for the Visually Impaired (ISVI) located at 658 East State Street, Jacksonville, Illinois. The project involves investigating and remediating flooding issues in Building 17 that occur during and after rainfall events. The vendor must conduct a comprehensive inspection to identify and address foundational issues causing water infiltration, including foundational cracks, failed or collapsed drainage tiles, and improper water shedding. All remediation work must comply with current safety standards, include removal and disposal of all debris and construction materials, and be completed by June 30, 2026. The project is expected to begin 30 to 60 days from notification of bid acceptance or as mutually agreed upon between the vendor and school staff. Work dates and hours must be coordinated with school staff to minimize disruption to the visually impaired students and staff on campus. The vendor must provide a five-year warranty on all completed work from the date of customer acceptance.

The vendor selected for this contract must be licensed and insured, responsible for securing all necessary permits, licenses, and utility locating services. Special safety considerations apply given the presence of visually impaired individuals on campus, requiring the vendor to establish barriers or temporary fencing around work areas as needed and coordinate closely with school staff. The vendor is fully responsible for all materials, equipment, supplies, and labor necessary to complete the work; restoring any damage to landscaping, sidewalks, and buildings; and maintaining daily safety and security cleanup at the end of each work day. The contract requires strict compliance with OSHA standards, environmental protection agency regulations, and all applicable state and federal laws. The Chief Engineer serves as the Contract Monitor and sole arbiter of vendor performance. An addendum notes that a previously collapsed clay pipe on the south side lawn near the building was not connected to the sanitary sewer system.
