About this file
This document describes a state and local contract opportunity issued by the Virginia Department of Health (VDH) - Greene County Health Department for the provision of building construction services, floor covering, and floor covering installation and removal equipment and supplies. The contract involves the removal of existing flooring and the installation of approximately 1,918 square feet of new vinyl flooring, including shoe-mold, over a consecutive four-day period. Mandatory site visits must be scheduled prior to submitting a bid, and all bids must be submitted through the eVA Quick Quote system by the due date of May 16, 2024. The contract period is from May 17, 2024 through June 20, 2024.
The contract is set aside for award priority to DSBSD-certified micro or small businesses, including women-owned and minority-owned businesses. The awarded vendor must provide a valid Certificate of Insurance before work can commence, and all-inclusive pricing, including shipping, fees, and warranty costs, must be provided. Payment will be made in accordance with the Commonwealth of Virginia Prompt Payment Act. The contract prohibits extra charges and requires prompt payment discounts, and subcontracting requires prior approval. Any damage to existing equipment or facilities must be repaired by the contractor at their own expense.

English
| Hierarchy tab | |
| Enter data on this sheet to define the master cost driver hierarchy for your site. | |
| The Level column represents the parent/child structure in which the data will be organized within the hierarchy. The data will appear in a tree structure within the UI based on the order it appears in this import file. | |
| Each Cost Driver must have a unique code which you define. A Cost Driver name can be repeated in the hierarchy, but each instance of that Cost Driver name must have a unique code. | |
| If you want to capture information for a cost driver beyond one price, populate the Detailed Grid column with a name, then add a tab to this file with a corresponding name. You will be able to configure a detailed grid using that tab, along with the columns defined in the Columns tab. |
| Columns tab | |
| The Columns tab is the dictionary of columns which can be used in the various detailed grids. | |
| You can create new columns for your detailed grids by completing this tab. |
Please note that codes of column must be unique.
Some data (column type, selector, aggregation ...) must comply with the list of proposed values.
Columns type : " list of choices ", all choices must be filled in the column dedicated and separated by ";".
Only one column should contain the total.
| Grids tab | |
| The Grids tab contains a list of all the grids that are used across all cost drivers. | |
| The Label of the grid defined on this tab must match a tab name in the file. |
| Other tabs | |
| Other tabs included in this file are for the detailed cost driver grids (one tab per grid). Each tab must match a name defined on the Hierarchy tab, as well as on the Grids tab. | |
| Use the column code to denote the column you want to appear in the detailed grid. | |
| The sequence the column codes appear on the tabs is the order in which they will appear in the grid in the UI. |
