About this file
This is a Past and Present Performance Questionnaire (PPQ) template for the Fire Department Vehicle Exhaust Inspection, Maintenance and Repair solicitation (FA930126Q0006). The questionnaire is to be completed by references or past customers who have worked with contractors bidding on this Air Force requirement and submitted with the complete solicitation package by 3:30 PM PDT on 27 August 2026.
The form requests general information about the contractor, the specific award being referenced (award number, dollar amount, and period of performance), and respondent contact details. Respondents must rate the contractor's past or present performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory) with consideration given to relevancy to fire department vehicle exhaust maintenance, inspections, and repair work. The scale emphasizes alignment between the referenced performance effort and the scope, magnitude, and complexities required by this solicitation. Completed questionnaires should be emailed to Contract Specialist Alejandra Negrete (alejandra.negrete@us.af.mil) or Contracting Officer Joshua P. Vasquez (joshua.vasquez.5@us.af.mil). The form includes space for additional comments to support performance ratings and requires a signature from the past performance point of contact.
