INSTRUCTIONS, CONDITIONS, AND NOTICES TO OFFERORS OR RESPONDENTS
INFORMATION TO OFFERORS (ITO) AND INSTRUCTIONS FOR PROPOSAL
PREPARATION
1.0 GENERAL INSTRUCTIONS
The offeror's proposal must include all data and information requested by this information to offerors and must be submitted in accordance with these instructions. The offer shall be compliant with the requirements as stated in the Solicitation and all attachments in Section J.
Non-conformance with the instructions provided in this information to offerors may result in an unfavorable proposal evaluation and removal from consideration for award.
1.1. INFORMATION TO OFFERORS
1.1.1. Point of Contact (POC). The primary POC for this acquisition is the Contracting Officer (CO), Mr. Wesley L. Marcik. Address any questions or concerns to the CO at commercial phone: (406) 731-4018 or email: wesley.marcik@us.af.mil. The secondary POC is Ms. Colleen T. Goad. Contact information for Ms. Goad is commercial phone: (406) 731-3473 or email:
colleen.goad.1@us.af.mil.
1.1.2. Discrepancies and Ambiguities. If an offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the offeror shall immediately notify the CO in writing with supporting rationale not later than 15 days after the Request for Proposal (RFP) release. Also, the Offeror shall give written notification to the CO of ambiguities in the solicitation, including the PWS, not later than 15 days after RFP release.
1.1.3. Revisions and Amendments. The Government reserves the right to revise or amend the specifications or the solicitation prior to the proposal closing time. Such revisions or amendments will be communicated by formal amendment (Standard Form 30) to this RFP. If such amendments require material changes in quantities or costs, the proposal closing date may be postponed by enough days to enable Offerors to revise their proposals. In such cases, the amendment will include an announcement of the new proposal closing date and time.
1.1.4. Pre-Performance Conference. A pre-performance conference will be scheduled by and held at the office issuing this RFP prior to the commencement of any work under the contract that will be awarded as a result of this solicitation and offer. The Contractor or his duly authorized representative should attend the pre-performance conference. NOTE: Due to COVID-19, there is a chance that the pre-performance conference will have to be conducted via tele-conference. More information will be provided after award.
1.1.5. Data Transmission. To facilitate review and evaluation for this source selection the Government proposes to transmit data via commercial systems. Distributed material will be identified as source selection sensitive and distribution strictly limited to appropriate personnel.
Should any contractor object to their proprietary information being shared between source selection evaluators and advisors via commercial e-mail as described above, please advise the secondary POC (Ms. Colleen T. Goad), at commercial phone: (406) 731-3473.
1.1.6. Competitive Range Determination. If a competitive range determination is necessary, the Government intends to limit the number of contract proposals to only the most highly rated proposals in accordance with Federal Acquisition Regulation (FAR) 15.306(c)(2).
1.1.7. Debriefings. The CO will promptly notify offerors of any decision to exclude them from the competitive range, whereupon they may request and receive a debriefing in accordance with FAR 15.505 and 15.506. The CO will notify unsuccessful offerors in the competitive range of the source selection decision in accordance with FAR 15.506. All offerors may request a debriefing IAW 15.505 or 15.506. If offerors are eliminated during the competitive range determination they may elect to wait until post award to receive their debriefing; however each offeror may only receive a total of one debriefing.
1.1.8. Electronic Reference Documents. All referenced documents for this solicitation are available on the new BETA SAM website at https://beta.sam.gov/. Potential offerors are encouraged to subscribe for real-time e-mail notifications when information has been posted to the website for this solicitation.
1.1.9 Site Visit. Due to the geographical expanse of this project, no site visit can/will be conducted.
1.1.10. Final Proposal Revisions (FPR). Once discussions, if required, have been concluded, the CO will request that offerors provide a FPR.
1.1.11. Questions Concerning the Solicitation. All questions concerning the solicitation must be submitted in writing not later than 2:00 p.m. (MT) on 12 June 2020. Submit questions to the secondary POC, Ms. Colleen T. Goad via email: colleen.goad.1@us.af.mil.
1.2. PROPOSAL PREPARATION INSTRUCTIONS
1.2.1. General Instructions and Information. These instructions prescribe the format of proposals and describe the approach for the development and presentation of the proposed data.
The Government intends to award this contract using the Lowest Price Technically Acceptable (LPTA) approach. Offers will be evaluated using the factors in Section M, Evaluation Factors for Award. The offeror’s proposal must include all data and information requested and must be submitted in accordance with these instructions.
1.2.1.1. This solicitation is being conducted in accordance with FAR 15.3, Source Selection and AFFARS 5315.3. Offers will be evaluated using the factors and subfactors under Section M, Evaluation Factors for Award. Non-compliance with the RFP may be grounds to eliminate the proposal from consideration for contract award.
1.2.1.2. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired. The proposal should not simply rephrase or restate the Government’s requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their facilities and experience, and will base its evaluation on the information presented in the offeror’s proposal.
1.2.1.3. The Offeror shall make a clear statement in Section A of the proposal documentation volume that the proposal is valid through 31 December 2020.
1.2.1.4. Proposals shall not contain classified information.
1.2.1.5. Alternate proposals will not be accepted.
1.2.1.6. In accordance with FAR Subpart 4.8 (Government Contract Files), the Government will retain one copy of all unsuccessful proposals. Unless the Offeror requests otherwise, the Government will destroy extra copies of unsuccessful proposals.
1.2.2. Format Instructions. This section provides general guidance for preparing proposals as well as specific instructions on the format and content of the proposal. Each volume shall be complete in itself. The proposal shall include all of the information requested in the subparagraphs. Failure to do so may adversely affect the evaluation. A proposal that is sufficiently documented in a complete, orderly, and detailed manner will enable the Government to perform a thorough and fair evaluation.
1.2.2.1. Proposal Volumes and Formatting. In order to mitigate risk of COVID-19 contagion via contact with physical materials, all proposals are to be received electronically in Adobe Acrobat PDF format via the DoD SAFE File Exchange system. Offers will consist of three (3) Volumes; Volume I Price/Contract Documents; Volume II Technical Requirements and Volume III Past Performance. For ease of evaluation, the required extent of data are described below.
1.2.2.2. Proposal Volume Organization, Number of Copies and Page Limitations. All offerors shall provide an original copy of their proposal on Adobe Acrobat PDF format. Offers will consist of all three volumes, each in a separate PDF file. The titles and contents should be as defined in this document, all of which shall be within the required page limits as specified in the table below. Total page count will be based on the proposal submission. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal.

Table of Contents and Glossaries do not count against the page limitations for their respective volumes.
1.2.2.6. Evaluation Notices (ENs) and Final Proposal Revisions (FPRs). Proposal format restrictions shall apply to responses to ENs and final proposal revisions.
1.3. RECEIPT OF PROPOSALS.
1.3.1. Volumes I (Price/Contract Documents), II (Technical Requirements) and III (Past Performance) ORIGINAL MUST be submitted electronically via the DoD SAFE File Exchange System website. (NOTE: indicating intent to submit a proposal doesn’t commit a potential offeror to submitting a proposal. However, the offeror must indicate intent by the timeline noted above).
1.3.1.2. The Contract Administrator shall, upon receipt of the intent e-mail, reply with a DoD SAFE Drop-Off request to the offeror’s email. The request will be valid for 14 calendar days.
1.3.1.3. The offeror, upon receipt of the Drop-Off Request e-mail, will open the DoD SAFE link, attaching the proposal files to the screen page. Proposals delivered in response to this RFP shall reflect the following information on the Comments section:

1.4. PROPOSAL VOLUME INSTRUCTIONS
1.4.1. Volume I - Price/Contract Documents (FACTOR 1). The purpose of this volume is to provide information to the Government to determine a fair and reasonable price and for preparing the contract document and supporting file. The offeror’s proposal shall include a signed copy of the Model Contract, Sections A (SF1442) through K, completed as required, respective financial data, and a completed Attachment 2b – Bid Schedule.
1.4.1.1. Price Information. The CO has determined there is a high probability of adequate price competition for this acquisition. Upon examination of the initial offers, the CO will review this determination and if, in the CO’s opinion, adequate price competition exists, no additional cost information will be requested and certification under FAR 15.406-2 will not be required. If however, at any time during this acquisition the CO determines that adequate price competition no longer exists offerors may be required to submit information necessary for the CO to determine the price fair and reasonable in accordance with FAR 15.403-5.
1.4.1.1.1 Insert proposed unit and extended prices in Section B for each Fixed Price Contract Line Item Number (CLIN). Offerors must propose on every contract line item, even if it is $0.00, in order to be considered for award. All CLINs/SubCLINs on the SF 1442 will count towards total evaluated cost.
1.4.1.1.2. Insert proposed unit and extended prices on Attachment 2b – Bid Schedule for each Unit Price and Est. Extended Amount for each item. Offerors must propose on every contract line item, even if it is $0.00, in order to be considered for award. The “Total Est. Extended Amount” for each period should match the corresponding CLIN in Section B to the nearest two decimal points.
1.4.1.1.3. All dollar amounts for all CLINs/SubCLINs should be proposed in whole dollar amounts rounded to two decimal points, for example: $1,423.78.
1.4.1.2. Contract Documents. As part of the proposal submission, include the following:
1.4.1.2.1. Section A - Completed Standard Form 1442 (Solicitation, Offer, and Award).
Completed form signed by the offeror constitutes an offer, which the Government may accept.
1.4.1.2.2. Completed Section K, Offeror’s Representations (Prime). Complete the FAR representations and certifications using the System for Award Management Application (SAM) at https://www.sam.gov. However, if any FAR or DFAR representations and certifications required are not included in SAM, submit the completed representation or certification as part of this chapter. Additionally, include any FAR, DFAR, or AFFARS clause requiring fill-in information, as well as any other acknowledgments required.
1.4.1.2.3. Acknowledgement of all amendments. Each offeror must sign and return one copy of the Standard Form (SF) 30 by the date specified on the form.
1.4.1.2.4. Statement of the period for which the proposal is valid. Ensure period specified is equal to or greater than the period stated in Block 13, Requirement D of the solicitation
(SF1442).
1.4.1.2.5. If a teaming arrangement is contemplated. Provide complete information as to the arrangement and relationship and copies of any teaming or joint venture agreements.
1.4.1.2.6. Authorized Offeror Personnel. Provide the name, title and telephone number and email address of the company/division point of contact regarding source selection decisions made with respect to your proposal and who can obligate your company contractually. Also, identify those individuals authorized to negotiate with the government.
1.4.1.2.7. Company/Division Street Address. Provide company/division's street address, county and facility code, and size of business (large or small).
1.4.1.2.8. Exceptions to Solicitation Requirements. By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Each exception shall be specifically related to each paragraph and/or specific part of the RFP to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule and cost and specific requirement of the RFP.
1.4.1.2.9. Financial Data. Submit financial data in this volume to include the following:
(i) Names address and points of contact for all financial institution organizations utilized to determine if the contractor has adequate financial resources to perform the contract or the ability to obtain them.
(ii) Written authority to contact the provided financial institution.
1.4.2. Volume II - Technical Requirements Documents (FACTOR 2). The offeror will provide documented proof of capability to perform the requirements of the contract by providing answers to the Evaluation Factors in Section M of the solicitation.
1.4.2.1. To be considered technically acceptable the offeror must provide rationale for all focus areas listed in Section M.
1.4.2.2. Offerors will be rated Acceptable/Unacceptable directly from the information they provide.
1.4.3. Volume III – Past Performance Documents (FACTOR 3).
1.4.3.1. Quality and Satisfaction Rating for Contracts Completed in the Past Five Years:
Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality of completed product for no more than three (3) relevant past efforts/contracts (similar in scope and magnitude of effort and complexities this solicitation requires). In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.
This information is required on the offeror and all subcontractors, teaming partners, and/or joint venture partners who are proposed to perform 25% percent or more of the proposed effort based on the total proposed price or perform aspects of the effort the offeror considers critical to overall successful performance. Offerors are cautioned that the Government will use data provided by each offeror in this volume and data obtained from other sources in the evaluation of past performance.
The government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this RFP. The government will use information submitted by the offeror and any other sources of information available to the Government to assess past performance.
Provide a list of no more than three (3), of the most relevant contracts performed for Federal agencies and commercial customers within the last five (5) years. Relevant contracts include projects directly related to directional boring to erosion repair. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Please completely fill out and submit Attachment 9: Past Performance Questionnaire on up to three recent contracts you consider most relevant to demonstrating your ability to perform the proposed effort.
Along with the information required in this paragraph, the offeror shall submit a consent letter (Attachment 6) executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the offeror so the offeror can respond to such information. For each identified effort for a commercial customer, the offeror shall also submit a client authorization letter, authorizing release to the Government of requested information on the offeror's performance.
1.4.3.2. Specific Content - Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort and to what aspects of the proposed effort they relate.
This may include a discussion of efforts accomplished by the offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk.
Merely having problems does not automatically equate to a limited or no confidence rating, since the problems encountered may have been on a more complex program, or an offeror may have subsequently demonstrated the ability to overcome the problems encountered. The offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions in terms of improvements achieved or problems rectified. This may allow the offeror to be considered a higher confidence candidate. For example, submittal of quality performance indicators or other management indicators that clearly support that an offeror has overcome past problems is required. Categorize the relevant information into the specific Technical subfactors and Cost/Price factor used to evaluate the proposal.
1.4.3.3. Organizational Structure Change History - Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
File details come from the government source that posted it. Updated .
