PAST PERFORMANCE QUESTIONNAIRE

NOTICE TO OFFERORS

Contained herein is the questionnaire offerors shall send to those firms that the offeror has identified in response to the solicitation requirements. This questionnaire may also be submitted for references of key personnel.

As stated in the solicitation, the Contracting Officer, in the interest of expediency, will start collecting past performance data before proposal receipt. Offerors are requested to submit their past performance information in advance of the proposal due date.

Offerors shall send questionnaires to their references and request the return of the questionnaire to the Contracting Officer as soon as possible prior to the established closing date of the solicitation. Offerors shall complete Blocks 1 and 2 of the questionnaire.

Exceptional (E) Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Very Good (VG) Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory (S)Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Marginal (M)Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions were only marginally effective or were not fully implemented.

Unsatisfactory (U) Performance does not meet most contractual requirements and recovery was not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions were ineffective.

For each question, please place an “X” in the box corresponding to the rating. Please provide supporting rationale for all ratings except “Satisfactory” . The narratives need not be lengthy, just detailed, clear and concise. Space for narrative comments is provided in Block 10. If more space is needed, use the back of the questionnaire or attach additional pages.
