1.3 ENTRY AUTHORITY LISTS FOR ELLSWORTH AIR FORCE BASE, SD (EAFB, SD).

A. Entry Authority List (EAL): These lists are kept at the gates to verify employment. The following guidelines apply:

1. Original EAL and two (2) copies to the Contracting Office are required.

2. The EAL must be in alphabetical order, on company letterhead, containing the complete address and phone number of the company, signed by an authorized company representative, and in the following format: Request the following personnel be allowed access to the base (include contract number, project number, location of project, and building number) in performance of our contractual requirements:

EMPLOYEES DRIVERS COMPANY

LAST NAME, FIRST, MI LICENSE # NAME

3. DO NOT combine your EAL with the subcontractor’s EAL. Each company/subcontractor must have a separate EAL.

4. Additions/Deletions of Personnel: Submit, immediately, any additions or deletions to the Contracting Officer. Include only changes to the current EAL.

5. Escorts: An asterisk (*) on the EAL by key personnel such as a foreman or supervisor will allow temporary entry of supply deliveries/pickup/repair. The escorts must accompany these personnel the duration of time on the installation. A maximum of three (3) key personnel per prime contractor is allowed. A phone number where the escorts can be reached must be included on the

EAL.

6. Approximately seven (7) calendar days are required for processing of EALs.

1.4 VISITOR’S PASS (Air Force Form 75): The visitor’s pass is required for all employees and vehicles. It has two parts, one for placement on the driver’s side of the dashboard (must be visible at all times), the other part is issued to the individual and must be on his/her person at all times. Each employee must obtain this pass by presenting his/her drivers license, vehicle registration and proof of insurance at the Visitor Control Center, located outside the main gate. If any of the above identification is not on the employees person, he/she will be escorted off of base. It is recommended that a magnetic sign be placed on the left door of the vehicles that do not have a company logo.

Periodic spot checks of drivers licenses and AF 75’s will be conducted on a random basis. It is the responsibility of each employee to turn in the pass at the depository (on driver’s side of the gate as you depart the base) after completion of the contract(s).

1.5 CONTRACTOR’S HAUL ROUTES: Access to the project will be by the Commercial Gate on Monday thru Friday and the Main Base Gate on Saturday, Sunday, and holidays, thence over the haul route indicated in the Delivery Order (DO). The designated routes shall be used by both loaded and unloaded vehicles. Contractors must immediately clean roads of any debris, mud or rocks originating from their vehicles. The contractors shall tarp or otherwise cover their trucks as necessary to prevent blowing debris.

1.6 WORK SCHEDULE:

A. Working hours for the contractor will normally be between the hours of 6:30 a.m. (7:30 a.m. in housing areas) and 5:00 p.m. excluding Saturdays, Sundays and Federal holidays.

B. If the Contractor desires to work during periods other than above, additional government inspection forces may be required. The Contractor must make his/her request to the Contracting Officer three days in advance of his/her intention to work during other periods to allow assignment of additional inspection forces. The Contracting Officer shall determine, in his/her discretion, whether such inspection forces are available and whether work will be allowed on a non-scheduled day. The Contracting Officer may authorize the Contractor to perform work during periods other than normal duty hours/days; however, if inspectors are required to perform in excess of their normal duty hour/days solely for the benefit of the Contractor, the actual cost of inspection at overtime rates will be charged to the contractor. These adjustments to the contract price may be made periodically as directed by the Contracting Officer.

1.7 ACCIDENT PREVENTION PLAN:

A. In performing this contract, the Contractor shall provide for protecting the lives and health of employees and other persons; preventing damage to property, materials, supplies, and equipment; and avoiding work interruptions. For these purposes, the Contractor shall --

2. Comply with the standards issued by the Secretary of Labor at 29 CFR Part 1926 and 29 CFR Part 1910.

3. Ensure that any additional measures the Contracting Officer determines to be reasonably necessary for this purpose are taken. Failure of the Contracting Officer to notify the Contractor of additional safety measures does not relieve the Contractor of liability and duty to prevent accidents by reasonable actions. The Contracting Officer shall notify the Contractor of any noncompliance with these requirements and of the corrective action required. This notice, when delivered to the Contractor or the Contractor’s representative at the site of the work, shall be deemed sufficient notice of the noncompliance and corrective action required. After receiving the notice, the

Contractor shall immediately take corrective action. If the Contractor fails or refuses to take corrective action promptly, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective action has been taken. The Contractor shall not base any claim or request for equitable adjustment for additional time or money on any stop order issued under these circumstances.

4. The contractor shall comply with Section 01500, Construction Facilities and Temporary Controls.

B. The Contractor shall comply with all pertinent provisions of the current U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1.

C. The Contractor shall maintain an accurate record of exposure data on all accidents incident to work performed under this contract resulting in death, traumatic injury, occupational disease, or damage to property, materials, supplies, or equipment. The Contractor shall report this data in the manner prescribed by the Contracting Officer.

D. The Contractor shall be responsible for its subcontractor’s compliance with these paragraphs.

E. Before commencing the work, the Contractor shall:
