About this file

This is an Annual Compliance Report for a state-funded Recovery Center, submitted by the vendor to the local Health Service Center (HSC) Liaison and Statewide Peer Support Representative (PSR) Director. The report is due annually by August 1st and covers a 12-month reporting period. The Recovery Center must maintain compliance with requirements outlined in RFP Section 2.03 Scope of Work, including operational standards such as serving individuals 18 years or older with mental illness, maintaining minimum 40 hours per week of operation as a drop-in center, employing a full-time Recovery Center Director and at least a 20-hour-per-week Recovery Coordinator certified as a Peer Support Specialist, and adhering to state policies regarding vehicle use and tobacco-free environments. Required staff qualifications include completed background checks and documented training, with expectations that leadership attend conferences annually.

Section 2 of the compliance report details required activities that directly impact payment to the vendor, including development of monthly event calendars distributed at least 10 days in advance, provision of at least one formal Peer Support group weekly, at least one Recovery Education group weekly, and at least one structured social or recreation activity daily. Additional requirements include monthly member meetings and director-liaison meetings with required documentation, provision of computers and internet access for staff and members, monthly social media marketing updates, and weekly data reporting. Failure to complete three or more required activities consecutively results in a warning letter requiring correction within 30 days, with potential disciplinary action including contract termination. Reduced payment occurs for months when activities are not completed.
