About this file

This is a Quarterly Compliance Report attachment for Recovery Center vendors operating under a state contract agreement. The report is designed to monitor vendor compliance with the Scope of Work outlined in RFP Section 2.03 and must be completed by Recovery Center Directors and submitted to the local liaison by the 15th of the month following each quarter (April 15th for Q1, July 15th for Q2, October 15th for Q3, and January 15th for Q4). Section 1 of the report addresses mandatory compliance requirements including member age verification, minimum operating hours of 40 hours per week, drop-in center operations, policy and procedure updates, adherence to the State Recovery Center Handbook, organizational chart changes, staffing requirements (full-time Director and part-time Coordinator certified as a Peer Support Specialist), background checks, staff training documentation, director and coordinator conference attendance, vehicle use policies, and tobacco-free environment maintenance. Non-compliance in Section 1 triggers a letter of warning with a 30-day correction period, after which disciplinary action up to and including contract termination may be imposed.

Section 2 of the report requires vendors to document completion of specific programmatic activities that directly impact payment, including monthly event calendar development submitted at least 10 days in advance, weekly peer support and recovery education groups, daily structured social and recreational activities, monthly member meetings, monthly meetings between the Recovery Center Director and liaison, provision of computers and internet access, monthly social media marketing updates, and timely weekly submission of statistical reports. Failure to complete Section 2 activities may result in reduced monthly payment, and failure to complete activities for three consecutive months triggers a letter of warning with the same 30-day correction and potential termination provisions as Section 1 violations.
