About this file

This is an Annual Compliance Report template for Recovery Centers operating under a state contract. The document serves as an accountability mechanism requiring Recovery Center Directors to complete and submit annual reports to local Health and Wellness Consortium (HSC) Liaisons and the Statewide Peer Support and Recovery (PSR) Director by August 1st each year. The report covers a 12-month period and addresses two primary sections of compliance requirements outlined in the RFP Section 2.03 Scope of Work. Section 1 encompasses mandatory operational and staffing standards, including requirements that all center members be at least 18 years old, centers operate minimum 40 hours weekly as drop-in facilities, staff maintain current certifications, and facilities comply with state policies regarding vehicle use and tobacco-free environments. Section 2 details required programmatic activities including monthly event calendars distributed at least 10 days in advance, weekly peer support and recovery education groups, daily structured social/recreation activities, monthly member meetings, monthly director-liaison meetings, provision of computers and internet access, monthly social media updates, and timely weekly data reporting.

Non-compliance carries escalating consequences beginning with a warning letter requiring immediate correction within 30 days, and progressing to disciplinary action up to and including contract termination. Specific staffing requirements include a full-time Recovery Center Director and a Peer Support Specialist-certified Recovery Coordinator working minimum 20 hours weekly. Vendors must maintain updated job descriptions, complete background checks meeting state requirements, provide documented required training, and ensure Director and Coordinator attendance at conferences. Failure to complete programmatic activities in Section 2 results in reduced monthly payment, with three consecutive months of non-compliance triggering formal warning procedures. The report is reviewed locally in QM meetings and provided to local leadership, establishing accountability across organizational levels.
