About this file
This is a Quarterly Compliance Report form for Recovery Center Directors operating under a state contract. The form must be completed by the 15th of the subsequent month following each quarter (April 15th, July 15th, October 15th, and January 15th) and submitted to the local liaison for forwarding to the Quality Management Committee. The Recovery Center must maintain compliance with requirements outlined in RFP Section 2.03 Scope of Work, including serving only members 18 years of age or older with identified mental illness, operating a minimum of 40 hours per week as a drop-in center, and maintaining a tobacco-free environment. Required staffing includes a full-time Recovery Center Director and a part-time Recovery Center Coordinator (minimum 20 hours per week) who must be certified as a Peer Support Specialist. The form requires documentation of organizational changes, updated job descriptions, completed background checks, and required staff training attendance.
Section 2 of the compliance report establishes required activities with payment contingent upon completion, including developing monthly event calendars at least 10 days in advance, offering at least one formal peer support group weekly, providing at least one recovery education group weekly, and scheduling at least one structured social or recreation activity daily. Additional requirements include holding documented member meetings monthly, conducting monthly meetings between the Recovery Center Director and liaison, providing computers and internet access for staff and members, updating social media marketing tools at least monthly, and submitting weekly statistical reports in a timely and complete manner. Failure to adhere to Section 1 compliance requirements results in a warning letter with 30 days to correct, with potential termination following non-compliance. Failure to complete Section 2 activities may result in reduced monthly payments, and three consecutive months of non-compliance triggers a warning letter with potential termination if not corrected within 30 days.
