About this file

This document is a Past Performance Questionnaire for a federal contract opportunity issued by the U.S. Department of Interior National Park Service (NPS) for Exhibit Planning, Design, Fabrication, and Installation at Cape Hatteras National Seashore, Bodie Island Visitor Center. The solicitation (140P2125R0034) uses a comprehensive performance evaluation process with two key forms: a Performance Rating Form and a Supplemental Comment Form.

The Performance Rating Form provides a detailed assessment framework with eight performance elements across four categories: Technical Performance, Timeliness of Performance, Quality/Customer Satisfaction, and Cost Control. Contractors will be rated on scales ranging from Outstanding to Unsatisfactory, with specific evaluation criteria for each element such as technical capability, understanding of requirements, resource utilization, communication, and cost effectiveness. The final question asks whether the evaluator would choose to do business with the contractor again, serving as an overall performance indicator. The document emphasizes that this information is source selection sensitive and cannot be used for promotional purposes.
