About this file

This is a Quality Assurance Surveillance Plan (QASP) for YCV-20 Maintenance and Repair FY26 under Department of Navy Solicitation N6264926RA034, serving as Attachment VI to the contract. The QASP establishes a comprehensive framework for Government surveillance of all maintenance and repair work performed on the YCV-20 vessel, ensuring compliance with technical, safety, administrative, scheduling, environmental, and security requirements specified in the Statement of Work. The plan requires monitoring of critical technical operations including docking/undocking, abrasive blasting, painting, welding/hot work, dry docking with certification and weight control, scaffolding, and threaded fastener installation, as well as administrative functions such as modification proposals, progress scheduling, and hazardous waste management.

Government oversight responsibilities are distributed among the Contracting Officer (overall contract authority), Contracting Officer's Representative/Program Manager (on-site inspections and document reviews), Marine Maintenance Officer and Government Docking Observer (technical oversight during operations and daily inspections), and Additional Experts including shipyard competent persons and environmental specialists (specialized inspections in gas-free certification, confined space entry, and hazardous waste management). Performance standards are evaluated using three ratings—Satisfactory (requirements fully met or exceeded), Marginal (minor deficiencies with corrective action taken), and Unsatisfactory (major deficiencies adversely affecting contract execution)—with an overall contract rating of Satisfactory or higher required at project completion. Surveillance methods include daily/weekly spot checks during active work phases, monthly formal progress meetings and safety briefings, inspections at key work phase milestones, and comprehensive final evaluations. All performance data will be documented and reported through the Contractor Performance Assessment Report System (CPARS), with additional quality assurance details provided in the Statement of Work.

Quality Assurance Surveillance Plan (QASP)

For

YCV-20 MAINTENANCE AND REPAIR FY26

Solicitation: N6264926RA034

Attachment VI

1. Purpose

This QASP establishes a comprehensive framework for Government surveillance of all aspects of work performed on YCV-20. Its purposes are to:

Ensure full compliance with all technical, safety, administrative, scheduling, environmental, and security requirements as set forth in the contract specifications defined in the Statement of Work (SOW).

Monitor and verify that technical operations—including docking/undocking, maintenance, abrasive blasting, painting, cleaning, ventilation, welding/hot work, dry docking (with certification and weight control), scaffolding, and installation of threaded fasteners—are executed per the approved plans.

Confirm that administrative functions (such as modification proposals, progress scheduling, authorized correspondence, integrated logistic support submittals, and hazardous waste management) are maintained and reported as required.

Record and document performance and initiate corrective action as needed when proper notification to the requiring activity and contracting officer, when deficiencies are discovered.

Provide the Government with a consistent method to evaluate overall contract performance and ensure that all deliverables are submitted in a timely, quality manner.

2. Government Roles and Responsibilities

Contracting Officer (CO): Holds overall authority for contract administration. Ensures that all contractual requirements (technical, safety, schedule, insurance, and environmental) are met.

Contracting Officer’s Representative (COR)/Program Manager (PM): Conducts on-site inspections and document reviews. Collects and analyzes documentation (e.g., inspection checklists, photographs, test reports). Monitors progress schedule updates and initiates

Corrective Action Requests (CARs) when necessary. Acts as the primary Government liaison.

Marine Maintenance Officer (MMO) & Government Docking Observer: Provide technical oversight during docking, undocking, and maintenance operations. Ensure vessel safety and protection of Navy interests during operations. Perform daily inspections, spot checks, and audits to verify work quality and compliance. Document all findings and assist in the CFR process.

Additional Experts (Shipyard Competent Persons, Environmental Specialists, and Security

Officers): Provide specialized inspections and services in areas such as gas-free certification, confined space entry, hazardous waste management, and physical security.

3. Performance Standards

Performance standards are established by referencing the detailed requirements in the contract

(Sections 1 through 9 of SOW). The standards encompass technical work, administrative procedures, safety and environmental safeguards, and secure document handling. Sample performance elements are provided in the following tables.

Performance

Overall performance must achieve a Satisfactory (or higher) rating in the

PM’s final report.

PM

At project completion

All performance elements rated Satisfactory or higher

4. Methods of Surveillance

The Government will monitor performance using the following methods:

On-site Inspections: The ACO and/or PM/COR will provide on-site oversight to assure the

Contractor is adequately performing to the contract specifications

Customer Feedback: Customer feedback may be obtained either from the results of formal customer satisfaction surveys or from customer complaints. Customer complaints, to be considered valid, must set forth clearly and in writing the detailed nature of the complaint, must be signed and must be forwarded to the contractor. The contractor shall maintain a summary log of all formally received customer complaints as well as a copy of each complaint in a documentation file.

5. Evaluation Ratings

Contractor performance for each element will be rated as follows:

Satisfactory (S): Requirements are fully met or exceeded.

Marginal (M): Minor deficiencies exist; prompt corrective action has been taken.

Unsatisfactory (U): Major deficiencies exist; performance adversely affects overall contract execution.

Overall contract performance must achieve a Satisfactory rating (or higher) in the PM’s final report.

6. Documenting Performance

All performance data will be documented by:

The Government will document positive and/or negative performance. Any report may become a part of the supporting documentation for any contractual action and preparing past performance information using the Contractor Performance Assessment Report System (CPARS).

7. Frequency of Measurement

Performance will be measured at the following intervals:

Daily/Weekly: Spot checks and on-site observations during active work phases.

Monthly: Formal progress meetings and safety/security briefings.

At Key Milestones: Inspections at specific work phase completions (e.g., after surface preparation, prior to dry docking, and at final inspection).

At Project Completion: Overall performance will be reviewed in the final evaluation.

As Needed: Additional assessments will occur when corrective actions or modifications are required.

NOTE: ANY ADDITIONAL QUALITY ASSURANCE OR QASP REQUIREMENTS AND RELATED

DETAILS ARE PROVIDED IN THE STATEMENT OF WORK (SOW), IN ACCORDANCE WITH THE
