About this file

This is Attachment S - Tie Breaking Certifications for RFP #10872 from the Florida Department of Juvenile Justice, which outlines the precedence order for breaking ties between equal proposals. The document details five specific designations that can be used as tie-breakers when proposals are equal in price, quality, and service: Drug Free Workplace certification (requiring completion of Attachment K), Certified Minority Business Enterprise (CMBE) status, Service Disabled Veteran's Business Enterprise certification, Florida Business designation, and Foreign Manufacturer with a Factory in Florida status.

The certification form requires respondents to check "True" boxes next to any applicable designations and provide supporting documentation if requested. For CMBE and Service Disabled Veteran's Business Enterprise designations, respondents must also provide their net worth amount. The form includes spaces for the respondent's name, title, company name, email address, telephone number, signature, and date. All certifications must comply with their respective Florida Statutes, including F.S. 287.087, 287.057(11), 295.187(4), 287.084, and 287.092.
