About this file
The document is a Request for Substitution/Exemption form from the City of Phoenix Public Transit Department for managing Disadvantaged Business Enterprise (DBE) subcontractor changes. The form is designed to be submitted to the department's Compliance Section (ptdcompliance@phoenix.gov) for approval before making any modifications to the proposed DBE utilization statement or monthly DBE utilization form. The form allows prime contractors to request exemptions such as releasing, substituting, changing the scope of work for, or reducing the amount allocated to a DBE subcontractor.
The form requires the prime contractor's authorized representative to provide details about the proposed changes, including the contract and project numbers, affected DBE subcontractors, and the specific scope of work. The prime contractor must also outline their good faith efforts in finding additional DBE participation. The Compliance Section will review the request and has the option to approve or deny the exemption, with the ability to revise the DBE goal and utilization percentage accordingly. The form includes sections for the contractor's signature, contact information, and a dedicated area for the Contract Compliance Specialist to document their decision and revised DBE metrics.
