About this file
This is a Past Performance Questionnaire template used to evaluate contractor performance on completed federal contracts during the proposal evaluation process for new solicitations.
The questionnaire collects reference information from government or company personnel who directly observed the contractor's work, including the reference's name, title, contact details, and affiliated agency or company. It requires documentation of the specific contract being evaluated, including contract number, total value, period of performance, and whether the contractor served as prime or subcontractor. The form establishes seven assessment elements rated on a five-point scale: Cost/Price Control (ability to manage costs and deliver at agreed prices); Schedule Adherence (timeliness of deliverables and reports); Performance (compliance with contract requirements and statement of work); Quality (adherence to quality assurance and performance-based requirements); Business Relations (cooperation, personnel stability, and responsiveness); Personnel Qualifications (ability to staff and retain qualified personnel); and Overall Performance Rating. Each element requires both a rating code (VG for Very Good, S for Satisfactory, M for Marginal, U for Unacceptable, or NA for Not Applicable) and supporting narrative. The questionnaire concludes with sections requesting identification of contractor strengths and weaknesses, a recommendation regarding future contract awards, and a certification statement signed by the reference affirming accuracy of the ratings and comments provided.
