About this file

This document is a California Civil Rights Laws Certification form required for state and local contract opportunities in California. The certification applies to all bidders and proposers executing or renewing contracts exceeding $100,000 on or after January 1, 2017, for RFQ 25-148 regarding Axon Body 4 Cameras, Accessories, and Warranties. Bidders must certify compliance with two key California civil rights statutes: the Unruh Civil Rights Act (Section 51 of the Civil Code) and the Fair Employment and Housing Act (Section 12960 of the Government Code). The form requires authorized representatives to provide their firm name, federal identification number, signature, printed name and title, date executed, and the county and state where execution occurs.

The certification also addresses discriminatory policies, requiring contractors to confirm that any internal policies against sovereign nations or peoples recognized by the United States government are not applied in violation of the Unruh Civil Rights Act or the Fair Employment and Housing Act. Certification is executed under penalty of perjury under California law. Failure to complete and submit this certification with a bid exceeding $100,000 may result in bid rejection or contract cancellation.
