EXECUTIVE SUMMARY
Powers Engineering and Inspection, Inc. was contracted by Weston Solutions to provide an API 653 Inspection for Tank 1 at the Creech Air Base in Indian Springs, NV.
The 18-ft dia. x 16-ft high was constructed in 1979 with a 0.250-in lap-welded bottom by Brown Tank and Steel. At the time of the inspection the tank was equipped with a steel cone roof and an aluminum honeycomb floating roof.
The report and executive summary are based on field inspection and evaluation in accordance with the API 653 Standard. Before the tank is returned to service all the requirements of API 650 and 653 should be met.
Inspection Methods
Employed Results
1 Identify any current leak paths resulting from corrosion, internal or external.
API 653
Appendix G MFL (Magnetic Flux Leakage) bottom corrosion survey.
100% Visual Inspection (VT) of shell, annular plate, corner weld and all bottom welds for defects cracks or corrosion Digital Camera Documentation.
1.1 The 0.250-in bottom was inspected for soil side corrosion utilizing the MFE 2412 Mark II scanner.
1.2 Isolated areas of magnetic flux leakage were detected during the floor scan and were proved up with a UT scrub. All of these indications were from grinded down weld tear-outs with the exception of one area of soil-side corrosion with a remaining thickness of 0.200-in. Next Inspection calculations show that if this soil-side corrosion is left unaddressed, this area will corrode to the minimum remaining thickness of 0.050-in in 21 years. No corrective action is required.
1.3 100% visual inspection of the bottom was performed. No significant product-side pitting was identified.
1.4 All areas inaccessible to the scanner were 100% UT scrubbed. No indication of soil-side corrosion was located.
1.5 The period-to-next formal API 653 Internal Inspection, utilizing the MRT method described in API 653 4th Edition may be extended to a maximum of 20 years per corrosion rate calculations if the bottom repairs are performed.
However, more stringent governing regulatory intervals of inspection may supersede this interval.
2 Identify any risk areas for future leak path development
Utilize Ultrasonic Testing (UT) to find areas of potential thinning or corrosion loss from internal corrosion.
Documentation of any brittle fracture concerns including nozzle weld spacing.
2.1 The external bottom extension-to-foundation caulking is in overall good condition with the exception of isolated voids and holes. It is possible that water has entered through these areas, is present under the tank and may contribute to accelerated soil-side corrosion. It is recommended that isolated caulking be applied to areas of the bottom caulking with voids.
2.2 The sump was UT scanned along equally spaced sections along the circumference with an average wall thickness of 0.400-in.
2.3 The 36-in manway cover is unable to fully open due to adjacent piping.
2.4 The fillet weld along the 24-in manway has isolated areas of under-fill. No corrective action is recommended.
2.5 The conduits along the tank shell are supported by uni-struts welded directly to the tank without the use of repads. No corrective action is recommended.
2.6 There are two areas along the shell with poor weld spacing (24-in manway repad weld and 4-in inlet nozzle repad weld – see Shell Roll Out drawing). Since the tank has been in operation at full fill height without an issue, no corrective action is required. Monitor these two areas for any change.
3 Settlement Survey of the shell base for settlement.
Survey of the bottom for settlement.
3.1 The maximum out-of-plane settlement was approximately 1.75-in, which is within acceptable limits of API 653 Appendix B calculations for differential settlement. See survey results.
3.2 No significant edge settlement was identified.
3.3 The bottom is cone-down by approximately 4-in.
3.4 The shell was inspected for verticality and distortion. No significant

Inspection Methods Employed
Results distortion was indicated.
3.5 The seismic loads were calculated utilizing the latest API 650 Appendix E (ASCE 7/ IBC 2009 methods). The risk of damage from a seismic event is minimal for all fill heights.
4 Venting VT of existing roof vents
4.1 The tank is equipped with 2: 12x24-in open roof vents.
4.2 The cone roof is also vented with a 6-in PV vent and an 8-in open vent.
5 Access Structure
VT of existing access structure.
5.1 The handrails and upper platform have only minor coating failure.
5.2 The caged vertical safety ladder supports are attached to the shell with seal-welded repads.
6 Cone Roof VT of existing roof
6.1 The tank is equipped with a steel cone roof.
6.2 The coating along the cone roof is in overall good condition with only minor, isolated areas of cracking type failure.
7 Floating Roof VT of existing floating roof
7.1 The tank is equipped with an aluminum honeycomb floating roof with a rubber, double wiper seal.
7.2 The floating roof primary seals are cracked throughout and have completely failed in isolated places. The secondary wiper was turned up in at least one place. It is recommended that both seals be replaced.
7.3 During time of inspection, the floating roof legs were set at approximately 60-in.
7.4 The rim space varies from 3-in to 7-in.
7.5 Trace product is present on the top of the floating roof deck. Verify that the tank is free from product before performing any hot work.
8 Tank gauging overfill protection
VT of existing systems Is a Clay valve present?
If the preferred gauging system present?
8.1 The 8-in and 4-in gauge poles are slotted along their entire height. Both gauge poles are equipped with seismic sleeves and seal-welded repads which allow vertical movement but restrain horizontal movement.
8.2 The tank is equipped with a Cameron tank radar which is mounted inside the 4-in slotted stilling well and does not appear to communicate with the Claval.
8.3 The Hi-Hi alarm is located at 16-in from the top of the shell and the Hi-alarm is located 48-in below the top of the shell. Both components are connected to a Claval.
8.4 Low and low-low alarms were not present.
8.5 The tank is not equipped with a level-float system.
8.6 The Hi-Hi alarm, as well as related pipe nipples and brackets, have threaded components which should be replaced with welded components.
8.7 A product recovery system was not observed near the tank.
9 Dike VT of the existing Dike
9.1 The tank is surrounded by a concrete dike with a liner installed throughout the dike area. Both the concrete pad and liner appear to be in good condition.

ACTION OR CRITICAL ITEMS
REQUIRED REPAIRS PRIOR TO RETURN TO SERVICE
(MANDATORY)
Listed below are the critical findings noted during the inspection that would prevent Tank 1 from returning to service:
1. (finding) Trace product was present on the top of the floating roof deck.
(recommendation) Verify that the tank is free from product before performing any hot work.
2. (finding) The rubberized components of the floating roof seal are dried and cracked and are no longer providing a reliable seal. In several areas, the seal is missing a section. (recommendation) Install new seals.
CONSIDERATIONS OR RECOMMENDED REPAIRS
RECOMMENDED REPAIRS PRIOR TO RETURN FOR SERVICE (NON-
MANDATORY/NEAR FUTURE)
The following are deficiencies identified during this inspection that the Inspector feels are mandatory items and should be addressed as soon as possible to insure continued safe operation of Tank 1:
3. (finding) An isolated area with 0.200-in remaining thickness-soil side was located on the floating roof deck. (recommendation) Install a 12x12-in seal-welded lap patch over the marked area (see Bottom Lay-Out Drawing).
4. (finding) The caulking along the foundation to bottom extension contact is in good shape with the exception of some isolated voids. (recommendation) Apply touch-up caulking to areas of the bottom-to-foundation caulking with voids.
5. (finding) The gaskets along the floating roof penetrations are worn or highly damaged.
(recommendation) Replace all gaskets along the floating roof penetrations.
6. (finding) No shell vent/overflows are present. Although a rim vent is present along the top angle, military guidelines may prefer shell vents. (recommendation) Install shell vents/overflows.
7. (finding) The tank is equipped with one 24-in manway and one 36-in manway. UFC requires two 36-in manways. (recommendation) Install two 36-in manways at ground level.
NON-MANDATORY ITEMS
8. (finding) No API 650 nameplate is present on the tank shell although one is located on the roof manway (recommendation) Install a properly mounted API650 nameplate above the 36-in manway.
9. (finding) A non API thickened insert was installed with poor weld spacing at an unknown time along the 4-in inlet nozzle. Presumably during installation of the insert, a slight inward distortion of the shell occurred. (recommendation) No corrective action is required because these items have been in service for some time, an indication that the brittle fracture risk is low.
BENICIA, CA 94510
Page 5 of 15 Date Printed: 3/8/13 1:24 PM
10. (finding) A poor weld spacing exists between a shell vertical weld and the 24-in manway repad weld. (recommendation) No corrective action is required because these items have been in service for some time, an indication that the brittle fracture risk is low.
