About this file

This document contains a Contractor Employee Biographical Data Sheet template used by USAID to collect personal and employment history information about individuals proposed to work on USAID contracts. The two-page form requests details like name, address, citizenship, education history, language proficiencies, references for previous three positions and consulting services, proposed salary and rationale, and certifications from both the employee and contractor. Instructions define the language proficiency levels and state the form is used to help USAID estimate support needs, qualifications, and determine salary reasonableness for cost reimbursement contracts. Individuals must provide the requested information, while contractors must certify steps taken to verify accuracy, with false statements subject to remedial action.

The related opportunity is for the USAID/Zambia Business Enabling Project, which aims to strengthen Zambian government and civil society organizations to implement improved policies and processes supporting rural enterprise development in trade, agriculture, energy, eco-tourism and natural resources. The project seeks to create an enabling environment incentivizing productive rural investments.
