About this file

This document is a Past Performance Questionnaire (PPQ) template used by the U.S. Government Publishing Office (GPO) Acquisition Services to evaluate contractor performance during source selection.

The form is completed in two sections: the contractor provides contract details (contractor name and address, project name, contract number, contract date, period of performance, pace of performance, completion date, contract value, contract type, and key project characteristics), while the evaluating organization representative completes the assessment portion (organization name and address, evaluator name, position, phone number, and months of performance monitoring). Evaluators rate contractor performance across nine dimensions using a standardized five-point scale: Exceptional (EX), Very Good (VG), Satisfactory (S), Marginal (MG), Unsatisfactory (US), or Not Observed (N/O). The evaluation categories include quality of products and services, overall performance, schedule adherence, technical requirements fulfillment, cost control, subcontracting compliance, customer satisfaction in routine matters, customer satisfaction during unexpected difficulties, and likelihood of repeat business. The form also requests narrative responses regarding contractor strengths and weaknesses, other completed projects, and average award fee percentages if applicable. PPQs are designated as source selection sensitive information under 41 U.S.C. 423 and must be included by offerors as part of their quote submissions, not sent directly to the contracting office.
