The 116th Air Combat Command is the only ANG unit operating the E-8C Joint Surveillance Target Attack

Radar System (STARS), an airborne ground surveillance and battle management aircraft. The Joint

STARS detects, locates, classifies, tracks, and targets ground movements on the battlefield, communicating real-time information through secure data links with United States command posts.

B. TANK INSPECTION COMMENTS

1. TANK CONSTRUCTION

Tank 2 is a welded, carbon steel, vertical, aboveground Jet-A (F-24) storage tank. The tank was constructed to API 650 standards in 1998 by Industrial Alloy Fabricators, Inc. The tank is 36’-8” in diameter and 40’-0” high with a nominal capacity of 7,143 BBL (300,000 gallons).

2. TANK FOUNDATION

According to facility personnel, Tank 2 rests on a concrete pad foundation. The foundation-to-containment floor joint sealant exhibits deterioration, disbondment, and vegetation growth, allowing for fluid to escape in the event of a spill. The chime-to-foundation joint sealant exhibits cracking and deterioration.

The tank is provided with twenty (20) seismic and wind anchors. The anchor bolts are distorted, warped, and may be under excess stress. An analysis should be performed to determine whether the anchor bolts are adequate. No shell deformations or signs of pipe stress were noted at the time of inspection.

A settlement evaluation was performed during the inspection. The settlement evaluation was satisfied based on data collected. See Appendix A for settlement results.

3. CATHODIC PROTECTION

The tank is not equipped with CP system. According to facility personnel, the tank is equipped with a concrete pad foundation, making CP unnecessary. No significant SS corrosion was observed during this inspection.

4. TANK BOTTOM

The tank bottom is of cone-down construction with a center sump. The tank bottom was examined using magnetic flux leakage (MFL), UTT, and VT methods. Five (5) UTT measurements were taken on each bottom plate. UTT readings were also taken circumferentially every 2-feet within 3-inches of the shell-to-bottom weld. UTT measurements indicated no significant signs of metal loss on bottom plates. VT was performed at bottom plate lap-welds and the bottom-to-shell weld. No significant findings were observed. VT was limited due to internal coating.

The MFL bottom scan was performed on accessible areas of the tank bottom to detect top-side (TS) and soil-side (SS) indications. The MFL bottom scan yielded four (4) TS and three (3) SS indications. TS pit depths range from 0.040-inches to 0.060-inches. All TS indications are coated, and corrosion is considered inactive. Follow-up UTT scans were performed at all SS indications. Remaining bottom plate thickness at indications ranged from 0.222-inches to 0.234-inches and have a remaining life greater than twenty (20) years.

The base plate for the Enraf ATG and manual gauging hatch exhibits two (2) pin holes at the weld, presenting a pathway for fluid ingress between the base plate and tank bottom, potentially accelerating bottom plate corrosion. The base plate is provided square corners, which are potential stress risers. No signs of stress cracking were observed at the time of inspection.

Internal piping for the receipt, main suction, low suction, and water draw-off are not provided with flanged connections at the shell penetrations IAW AW Standard Design. It should be noted a through-hole was observed on the main suction internal piping adjacent to the shell in the 12 o’clock position. The main suction piping extends to the tank sump and is not provided an anti-vortex plate. The main suction should extend half the radius of the tank to avoid issuing water during operations while an anti-vortex plate controls velocity of the suction line and avoids cavitation.

Internal piping for the main suction, low suction, and receipt is equipped with adjustable supports and are not IAW AW standard design. Piping for the water draw-off is also equipped with non-standard pipe supports. Piping for the main suction and low suction should be provided U-bolt pipe supports with

Teflon wrapping. Three (3) supports for the main suction and two (2) for the low suction are not in contact with the pipe or provide proper support. Pipe support base plates are not fully seal welded to the tank bottom, presenting a pathway for fluid ingress between the base plate and tank bottom, potentially accelerating bottom plate corrosion. No visual signs of corrosion were observed at the time of inspection.

Pipe support base plates are also provided with square corners. One (1) pipe support base plate for the water draw-off is welded ½-inch from the adjacent bottom plate weld. API requires 2-inches between adjacent welds to compensate for stress risers in the heat affected zone.

The internal floating roof (IFR) anti-rotation cable bottom attachment is not provided a bearing plate and is welded directly to the tank bottom. No visual signs of crevice corrosion were observed at the time of inspection.

Refer to Drawing A-1, “Bottom Layout”, in Appendix A for bottom plate locations, Table 1, “Bottom UTT

Measurements”, in Appendix A for bottom plate UTT measurements, Table 2, “Bottom Circumferential

UTT Measurements”, for bottom plate circumferential UTT measurements, and Table 6, “Bottom

Indication Calculations”, for the bottom indication remaining thickness and remaining life calculations.

5. TANK SHELL

The tank shell is equipped with five (5) fully coated, butt-welded, carbon steel courses. Shell plates were examined using UTT and VT methods. Nine (9) UTT measurements were taken on each plate of the 1st shell course. Ten (10) UTT measurements were taken on each of the 2nd through 5th shell courses using the spiral staircase for access. UTT measurements indicated no significant signs of metal loss on shell plates.

Tubing from the receipt HLCV to the float pilot is not provided conduit supports and is resting directly on tank anchor chairs, potentially accelerating coating failure and corrosion. No contact point corrosion was observed at the time of inspection. Tank 2 is not provided an emergency responder diamond IAW

NFPA 704. Emergency responder diamonds are the established standard system for identification of hazards materials present for emergency responders. Strapping charts are provided for Tank 2, however, current strapping charts are in 1/8-inch increments, whereas UFC mandates strapping charts be in 1/16-inch increments.

An out-of-roundness survey was conducted on the internal tank shell. The largest deviation from the tank nominal diameter was 2 11/16-inches at 3-feet above the shell-to-bottom weld. The maximum allowable deviation more than a foot above the shell-to-bottom weld per API 653 Table 10.2 is 3-inches.

Refer to Drawing A-2, “Shell Layout”, in Appendix A for shell plate locations, Table 3, “Shell UTT

Measurements”, and Table 9,”Shell Out-of-Roundness Measurements”, in Appendix A for shell plate UTT and out-of-roundness measurements.

6. FIXED ROOF

The tank fixed roof was examined using UTT and VT methods. Five (5) UTT measurements were taken on each fixed roof plate. UTT measurements indicated no significant signs of metal loss on fixed roof plates. One (1) conduit support on the fixed roof is not provided a base. Minor contact point corrosion was observed at the support-to-roof interface.

Refer to Drawing A-3, “Fixed Roof Layout”, in Appendix A for fixed roof plate locations and Table 4, “Fixed

Roof UTT Measurements”, in Appendix A for fixed roof plate UTT measurements.

7. TANK APPURTENANCES

a. TANK SHELL

Nozzles and appurtenances associated with the shell were inspected using UTT and VT methods.

Four (4) UTT measurements were taken on each nozzle with a welded connection greater than 1-inch at four (4) quadrants (0°, 90°, 180°, and 270°). One (1) UTT measurement was taken on each associated reinforcement pad.

Tank 2 is equipped with thirteen (13) shell nozzle penetrations: two (2) 36-inch manways, one (1)

¾-inch water draw-off, one (1) 8-inch receipt, two (2) 2-inch HLCV float pilot penetrations, two (2)

2-inch HLA/HHLA penetrations, two (2) 2-inch LLA penetrations, one (1) 24-inch x 24-inch flush clean out, one (1) 12-inch main suction, and one (1) 4-inch low suction.

The main suction, low suction, and receipt nozzle penetrations are constructed of stainless-steel and the water draw-off penetration is constructed of galvanized steel. Dissimilar metals welded directly together creates a galvanic corrosion cell. This configuration can potentially accelerate corrosion at the carbon steel shell. The flush clean out is not provided with a davit arm to facilitate handling of the cover plate. Manway 1 is provided a hinge rather than a davit arm with grease fitting. Reinforcement pad tell-tale holes for each manway, the flush clean out, main suction, and low suction were not installed at the horizontal center line of each nozzle. The tank is not provided with an independent product saver tank (PST). Tank 2 shares a PST with Tank 1, however, there is no method of testing level alarms or HLCV float pilot through the current configuration. Piping to Tank 1 PST consists of threaded connections. Threaded connections are no longer used in this application as they are a potential leak path.

Refer to Drawing A-2, “Shell Layout”, in Appendix A for shell nozzle locations and Table 5, “Nozzle/Repad UTT Measurement”, in Appendix A for nozzle and associated reinforcement pad UTT measurements.

b. INTERNAL FLOATING ROOF

The IFR was examined using VT methods. No significant observations or deficiencies were observed.

The IFR foam log seal was replaced with a double wiper seal during this OOS inspection. Stilling well

IFR penetration vapor seals, IFR leg support shoes, and an upgraded pressure/vacuum vent were also provided during the OOS inspection.

c. FIXED ROOF

Nozzles and appurtenances associated with the fixed roof were inspected using UTT and VT methods.

Four (4) UTT measurements were taken on each nozzle with a welded connection greater than 1-inch at four (4) quadrants (0°, 90°, 180°, and 270°). One (1) UTT measurement was taken on each associated reinforcement pad.

The fixed roof is provided fourteen (14) nozzle penetrations: two (2) 3-inch IFR grounding cables, two (2) 30-inch x 36-inch inspection hatches, one (1) 2 ½-inch Varec TLG, three (3) 12-inch temperature sensors, one (1) 24-inch center vent, one (1) 2 ½-inch IFR anti-rotation cable, one (1)

4-inch Vito temperature/water probe, one (1) 6-inch Enraf ATG, one (1) 6-inch manual gauging hatch, and one (1) 72-inch by 24-inch IFR installation hatch.

External coating on the Vito temperature/water probe, IFR installation hatch, temperature sensors, and fasteners exhibit flaking and base metal exposure with surface corrosion. Two (2) IFR grounding cables and one (1) IFR anti-rotation cable are provided with machined holes, allowing water ingress to the internal tank. The manual gauging hatch cover is not provided a latch to secure the cover while not in use. Confined space labeling on the two (2) inspection hatches exhibit fading and peeling. Enraf

ATG fasteners exhibit corrosion and one (1) fastener is missing. One (1) fastener for one (1) temperature sensor nozzle is not fully engaged. Nozzle and appurtenance fasteners should be present and fully engaged per manufacture requirements.

Refer to Drawing A-3, “Fixed Roof Layout”, in Appendix A for fixed roof nozzle locations and Table 5, “Nozzle/Repad UTT Measurement”, in Appendix A for nozzle and associated reinforcement pad UTT measurements.

d. GAUGING

The tank is provided a manual gauging hatch for manual gauging and sampling operations. An Enraf

ATG and Vito temperature/water probe are provided for automatic tank gauging. A mechanical Varec

TLG is provided for monitoring product level from the ground.

Stilling wells for the manual gauging hatch, Enraf ATG, and Vito temperature/water probe are not fully slotted. Fully slotted stilling wells allow for accurate gauging measurements during filling and dispensing operations. The Enraf ATG and manual gauging hatch are not provided with datum plates to establish a gauging zero point. The Vito temperature/water probe is attached to the tank bottom by a threaded hex nut and stud and is located at the periphery of the tank. The stilling well should not be attached to both the tank bottom and fixed roof as it can potentially cause damage due to settlement and should be located as close to the sump as feasible for accurate water gauging.

e. LEVEL ALARMS

The tank is provided with independent, shell mounted alarms in the low, high, and high-high positions as well as a HLCV float pilot for overfill protection. According to facility personnel, all level alarms and the HLCV float pilot are functioning properly. A low-low level alarm (LLLA) system is not provided for the tank; however, facility personnel confirmed that wiring conduit and alarms in the pump house have been installed in anticipation for an updated alarm chamber to include a LLLA. The level alarms are provided with threaded connections on associated piping. Threaded connections are no longer used as they are a potential leak path.

f. STAIRS AND PLATFORMS

The tank is provided a spiral staircase with one (1) upper platform for fixed roof access, one (1) work platform for access to the HLA / HHLA / HLCV float pilot, and one (1) mid-platform for access to

Manway 2. The HLCV float pilot extends into the path of the spiral staircase, allowing 24 ½-inches clearance between the appurtenance and staircase railing. OSHA mandates a minimum 26-inches clearance between appurtenances and staircase railings.

g. GROUNDING AND BONDING

The tank shell is equipped with four (4) grounding cables in good overall condition with no signs of detachment, deterioration or fraying. Grounding cable foundation penetration sealant exhibits voids and deterioration, allowing for fluid to escape in the event of a spill.

8. TANK AND PIPING COATING

External coating for the tank shell, fixed roof is an epoxy-polyamide with urethane topcoat. External tank shell and fixed roof coating exhibits intermediate coating exposure and widespread chalking, indicating the coating system is near the end of its life cycle. Prior to recoat, it is recommended to perform a coating condition survey, including an adhesion test, or review previous coating condition surveys to determine whether a full recoat is required or if a topcoat reapplication will suffice.

Piping provided for Tank 2 is constructed of stainless-steel and not provided with coating.

For TS corrosion protection, the tank bottom is provided with an epoxy-polyamide internal coating system. Internal coating is in good condition with no signs of damage or deterioration.

A DFT survey was performed on the external and internal tank shell, fixed roof, and tank bottom. Refer to Table 8, “Tank DFT Measurements”, in Appendix A for tank shell and fixed roof DFT results.

9. TANK PIPING SYSTEM

Piping within the containment dike is constructed of stainless steel. Flow arrows for the receipt and main suction piping exhibit fading and peeling. Thermal relief valves (TRV) at the receipt and main suction are in vapor service and associated crossover piping is provided with sight flow indicators. The low suction is provided with a sight flow indicator. Threaded connections and sight flow indicators are no longer used in this application as they are potential leak paths. The receipt is not provided with a double block and bleed (DB&B) valve upstream of the HLCV to permit isolation of system components for maintenance and repair.

10. FIRE PROTECTION SYSTEM

For internal fire protection, an aluminum honeycomb IFR is provided.

11. SECONDARY CONTAINMENT

Tank 2 is located within a 90-foot long by 90-foot wide concrete secondary containment with containment dike walls 6-feet 6-inches high. The containment dike is capable of holding approximately

393,850 gallons of liquid, satisfying the 1-foot UFC capacity requirement. Sealant for the dike walls and floor control joints exhibits deterioration, disbondment, and shrinkage, allowing a pathway for fluid to escape in the event of a spill. Widespread cracking was observed within the secondary containment dike.

A hydrostatic test was performed on the containment drain. The drain was filled with water and allowed to stand for 24 hours. At the end of the 24-hour period, there was no water loss observed, confirming the containment drain will properly hold fluid in the event of a spill.

C. FINDINGS AND RECOMMENDATIONS
