About this file
This document is a Past Performance Questionnaire for a Library of Congress (LOC) contract opportunity for Painting Frame Fabrication Services (Solicitation Number: 030ADV26Q0046). The questionnaire is designed to evaluate a contractor's performance across ten key dimensions, including contract management, product quality, project effectiveness, schedule compliance, reporting timeliness, personnel competence, communication, cost performance, and overall project execution.
The solicitation seeks a highly experienced museum exhibition Framer to create custom, conservation-grade frames, acrylic glazing, and support components for paintings on canvas. The contractor will be responsible for fitting paintings into frames on-site at LOC, attaching hanging and security hardware, with LOC handling the final exhibit installation. The questionnaire must be completed and returned by noon (12:00 PM Eastern) on February 2, 2026, to Eric Pacheco and Ramon Samuel, with the completed form providing a comprehensive assessment of the contractor's past performance using a rating scale from Unsatisfactory to Outstanding.

PAST PERFORMANCE QUESTIONNAIRE

Good
Performance meets contractual requirements and exceeds some to the Government's benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory
Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal
Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.
Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor's corrective actions appear or were ineffective.
2. Please provide narrative explanations for any Marginal, Unsatisfactory or No ratings and supplement your knowledge of the contractor’s performance with judgement of others in your organization.
3. The contractor submitted a proposal to the Library for the provision of audio visual and interactive experience design services. Past performance is of considerable importance to this acquisition. Therefore, input from current or previous customers of the contractor is very important.
