About this file
This is a Conflict of Interest Affidavit document required by the State of Maryland for the Electrical Maintenance & Repair Services solicitation (D7ELEC31SB) issued by the State Highway Administration (MDOT SHA). The affidavit requires bidders to disclose any conflicts of interest that would impair their ability to render impartial assistance to the State or create unfair competitive advantages. Bidders must warrant that no relevant facts or circumstances create conflicts of interest, except as disclosed in the affidavit. The document applies to all offerors, contractors, consultants, subcontractors, and their employees or agents who have authority to control or supervise work on the contract. The affidavit must be completed and submitted with the bid by the October 15th, 2026 bid closing date at 12:00 PM Local Time.
The affidavit requires bidders to immediately disclose in writing to the procurement officer (Tyler Zimmerman, TZimmerman@mdot.maryland.gov, 410-848-6565) any actual or potential conflicts of interest that arise after the affidavit date, including a description of actions taken or proposed to mitigate or neutralize the conflict. The authorized representative must sign the affidavit under penalty of perjury, certifying that all contents are true and correct. This document is a standard compliance requirement referenced in Maryland's Annotated Code, Metropolitan Area Regional Servicing (COMAR 21.05.08.08), and applies to all bidders responding to the electrical maintenance and repair services invitation for bids.
