About this file

This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to the Core Measures Vendor procurement for San Mateo Medical Center. The document requires all contractors to comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all associated DHHS regulations, guidelines, and interpretations. Contractors with 15 or more employees must designate a Section 504 Compliance Coordinator and provide that individual's name and contact information. The assurance is binding on the contractor, its successors, transferees, and assignees, and serves as a condition for contract award. Contractors must certify the accuracy of their compliance information through an authorized official's signature and title.

The document includes a provision for contractors with fewer than 15 employees, allowing them to refer disabled persons seeking services to other accessible providers if achieving facility accessibility would require significant alterations to existing facilities. This compliance assurance must be completed and submitted as part of the proposal package, demonstrating the contractor's commitment to non-discrimination and accessibility requirements under federal law. The document does not specify financial penalties or remedies but establishes this assurance as a mandatory prerequisite for contract eligibility.
