About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to contractors entering into agreements with the county. The document requires contractors to certify compliance with Section 504 of the Rehabilitation Act of 1973 and all applicable Department of Health and Human Services (DHHS) regulations. Contractors must complete the assurance form by indicating their employee count and, if applicable, designating a Section 504 compliance coordinator. The assurance is binding on the contractor, its successors, transferees, and assignees, and must be signed by an authorized official.
The document contains two primary compliance pathways based on organization size: contractors employing fewer than 15 persons are subject to an exception allowing referral to other accessible providers if facility modifications would constitute significant alterations, while contractors employing 15 or more persons must designate a compliance coordinator responsible for ensuring adherence to DHHS regulations. The form requires completion of contractor name, address, coordinator name and title, and certification by an authorized official with signature, title, and date. This assurance is a prerequisite for contract award and execution with San Mateo County.
