About this file

This is an Assurance of Compliance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, outlining Section 504 of the Rehabilitation Act of 1973 compliance requirements for contractors. The document serves as a binding agreement whereby contractors commit to comply with Section 504 regulations, all applicable Department of Health and Human Services (DHHS) requirements, and related guidelines and interpretations. The assurance applies to all contracts extended after the document's date and remains binding on the contractor, its successors, transferees, and assignees. Contractors must complete and sign the assurance form, providing authorized official signatures, title, and date of certification. The form includes a checkbox provision distinguishing between contractors employing fewer than 15 persons and those employing 15 or more persons, with the latter required to designate a Section 504 compliance coordinator and provide that person's name and the contractor's address information.

The document includes an exception clause acknowledging DHHS regulations that permit contractors with fewer than 15 employees to refer disabled persons to alternative accessible service providers if compliance with facility accessibility regulations would require significant alterations to existing facilities. Contractors must certify that all information provided is complete and accurate to the best of their knowledge. This compliance assurance applies to the FOHC Lobbies AV contract opportunity, an audiovisual system installation project for San Mateo Medical Center involving display replacement across three floors with a three-year term. All successful contractors must meet extensive regulatory requirements, including Section 504 compliance, HIPAA Business Associate obligations for personnel accessing protected health information, and corporate compliance standards, ensuring accessibility and non-discrimination in service delivery throughout the contract period.
