About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to contractors seeking to work with the county. The assurance requires contractors to comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. The document applies to all contracts awarded after the date of the assurance and is binding on the contractor, its successors, transferees, and assignees. The assurance must be signed by an authorized official and includes contractor identification information such as name, street address, and contact details.
The document contains two compliance pathways based on contractor size: contractors employing fewer than 15 persons are subject to an exception allowing referral to other accessible service providers if facility modifications would constitute a significant alteration, while contractors employing 15 or more persons must designate a specific person to coordinate compliance efforts in accordance with 45 C.F.R. 84.7(a). The contractor must certify the accuracy and completeness of the submitted information and provide the signature, title, and date of authorization from the responsible official. This assurance is a prerequisite requirement for contract consideration with San Mateo County and the County's Hospital system (San Mateo Medical Center).
