About this file

This is an Assurance of Compliance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing requirements for contractors to comply with Section 504 of the Rehabilitation Act of 1973, as amended. The document requires all contractors entering into agreements with the County to demonstrate compliance with Section 504 and all applicable Department of Health and Human Services (DHHS) regulations, including guidelines and interpretations issued pursuant to the statute. The assurance is binding on the contractor, its successors, transferees, and assignees, and serves as a condition for obtaining County contracts executed after the assurance date. Contractors must certify their compliance status and provide authorized signature documentation confirming the accuracy of submitted information.

The document contains a checkbox provision distinguishing compliance requirements based on contractor size: contractors employing fewer than 15 persons have streamlined requirements, while those employing 15 or more persons must designate a specific Section 504 coordinator responsible for coordinating compliance efforts. For smaller employers, DHHS regulations provide an exception allowing referral of disabled persons seeking services to alternative accessible providers if facility modifications would constitute a significant alteration. The assurance form requires submission of the contractor's name, address, designated 504 coordinator name (if applicable), authorized official signature, title, and date of certification, ensuring clear accountability and documented commitment to non-discrimination compliance throughout the contract period.
