About this file

This document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, relating to Section 504 of the Rehabilitation Act of 1973. The form is a compliance document that requires contractors to agree to adhere to non-discrimination regulations for individuals with disabilities, specifically focusing on facility accessibility and service provisions. Contractors must indicate whether they employ fewer than or 15 or more persons, and if the latter, they must designate a person to coordinate efforts to comply with DHHS regulations.

The form provides flexibility for smaller organizations (those with fewer than 15 employees) by allowing alternative methods of compliance, such as referring individuals with disabilities to other accessible service providers if significant facility alterations are not feasible. The document serves as a legally binding assurance that extends to the contractor's successors, transferees, and assignees, requiring an authorized official to certify the completeness and accuracy of the information provided. While directly related to the San Mateo Medical Center's After-Hours Pharmacy Remote Order Entry Services procurement, this specific attachment focuses on ensuring equal access and non-discrimination in service delivery.
