About this file
This is an Assurance of Compliance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing Section 504 of the Rehabilitation Act of 1973 compliance requirements for contractors seeking contracts with the county. The document requires contractors to agree to comply with Section 504 and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. Contractors must certify their compliance status by completing the form, which includes identification of a designated 504 compliance coordinator for organizations employing 15 or more persons. The assurance is binding on the contractor, its successors, transferees, and assignees, and is executed by an authorized official under penalty of certification.
The document distinguishes between contractors employing fewer than 15 persons and those employing 15 or more persons, with the latter required to designate a specific person responsible for coordinating compliance efforts. A notable exception permits contractors with fewer than 15 employees to refer disabled persons to other accessible service providers if facility accessibility modifications would constitute a significant alteration to existing facilities. The assurance form requires completion of contractor name, street address, city, state, zip code, designated 504 person's name, signature, title, and date of authorized official, serving as a prerequisite certification for contract award with San Mateo County.
