About this file

This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to the After-Hours Pharmacy Remote Order Entry Services contract opportunity. The document requires all contractors to comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. Contractors must certify their compliance status by completing the assurance form, which includes designation of a Section 504 coordinator for organizations employing 15 or more persons. The contractor must provide the name of the designated 504 person, contractor name, street address or P.O. box, city, state, and zip code, along with the signature of an authorized official and the date of certification.

Organizations employing fewer than 15 persons are exempt from designating a Section 504 coordinator but remain subject to the accessibility requirements of the regulation. However, DHHS regulations provide an exception allowing recipients with fewer than 15 employees to refer disabled persons seeking services to other accessible providers if compliance would require significant alterations to existing facilities. This assurance is binding on the contractor, its successors, transferees, and assignees, and the signatory confirms authorization to execute the document on behalf of the contracting entity.
