About this file
This document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, specifically relating to Section 504 of the Rehabilitation Act of 1973, as amended. The form is a standard compliance document that contractors must complete to demonstrate their commitment to non-discrimination and accessibility requirements for individuals with disabilities when seeking contracts with the county.
The form requires contractors to indicate whether they employ fewer than or 15 or more persons, with those employing 15 or more required to designate a specific person to coordinate efforts to comply with DHHS regulations. The document includes provisions that allow organizations with fewer than 15 employees to refer handicapped persons to alternative accessible service providers if making significant facility alterations is not feasible. This form is part of the broader contracting process for the San Mateo Medical Center's courier services contract, which has an estimated budget of $2,650,000 and serves the healthcare needs of San Mateo County residents.
