About this file

This is a Section 504 Compliance Assurance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to contractors seeking contracts with San Mateo County. The document serves as a binding compliance certification in which contractors attest to their adherence with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations and guidelines. Contractors are required to designate a compliance coordinator if they employ 15 or more persons, with smaller contractors employing fewer than 15 persons having limited accessibility requirement obligations under an exception clause. The form requires contractor signature, authorized official title, and date of certification, with the assurance binding on the contractor, its successors, transferees, and assignees.

The compliance assurance applies to all contracts executed after the date of certification and establishes that San Mateo County will award contracts in reliance upon the contractor's representations and commitments regarding disability non-discrimination standards. Contractors with fewer than 15 employees may refer disabled individuals to alternative accessible service providers if facility alterations are deemed economically infeasible. This document functions as a prerequisite contractual requirement, ensuring that all vendors providing services to San Mateo Medical Center and other county entities maintain compliance with federal disability accessibility standards throughout the contract performance period.
