About this file
This document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, related to Section 504 of the Rehabilitation Act of 1973. The form requires contractors to verify their compliance with federal disability non-discrimination regulations, specifically detailing how organizations with 15 or more employees will coordinate efforts to meet DHHS (Department of Health and Human Services) accessibility requirements.
The form provides an option for contractors to indicate their employee count (fewer than or 15 or more) and, if applicable, designate a specific person to coordinate compliance efforts. An exception clause notes that organizations with fewer than 15 employees who cannot make facility modifications may alternatively refer disabled individuals seeking services to other accessible providers. The document requires the authorized official to complete contact information, certify the accuracy of the submission, and sign the form, serving as a formal commitment to non-discrimination and accessibility standards.
