About this file

The document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, related to Section 504 of the Rehabilitation Act of 1973. The form requires contractors to verify their compliance with accessibility regulations for individuals with disabilities, specifically detailing requirements for organizations with fewer than 15 or 15 or more employees.

The form provides a template for contractors to certify their commitment to meeting DHHS (Department of Health and Human Services) regulations, with an option for smaller organizations (fewer than 15 employees) to refer disabled individuals to alternative accessible service providers if they cannot make significant facility alterations. The document includes blank fields for contractors to input their organization's name, a designated Section 504 compliance coordinator, address, and an authorized official's signature, affirming their understanding and agreement to non-discrimination and accessibility requirements.
