About this file

This is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing Section 504 compliance requirements for contractors bidding on county contracts. The document requires contractors to affirm their compliance with Section 504 of the Rehabilitation Act of 1973 and all applicable Department of Health and Human Services (DHHS) regulations. Contractors must indicate whether they employ fewer than 15 persons or 15 or more persons. Those employing 15 or more persons are required to designate a Section 504 coordinator responsible for coordinating compliance efforts with DHHS regulations. The assurance binds the contractor, its successors, transferees, and assignees, with authorized officials required to certify the accuracy of submitted information by signature.

The form includes an exception provision allowing recipients with fewer than 15 employees to refer disabled persons seeking services to other accessible providers if no method of compliance with facility accessibility regulations exists without significant facility alterations. All contractors must provide complete contractor identification information, including the designated 504 coordinator's name (if applicable), street address or P.O. Box, city, state, and zip code. Signature, title of authorized official, and date are required on the certification statement, with the contractor confirming the accuracy of all provided information.
