About this file
This is a Commercially Useful Function (CUF) Compliance Form required by the State of California under Military and Veterans Code 999.5 and Government Code 14837.4(A/B) for RFQ 25-128 regarding Applied Biosystems 3500 Genetic Analyzers HID or equivalent. The form applies to all solicitations where the prime bidder or any subcontractor holds small business (SB), micro business (MB), or disabled veteran business enterprise (DVBE) certification. Bidders and subcontractors must provide a written, signed statement detailing their specific role, services, and goods to demonstrate compliance with CUF requirements for the contract.
The form requires respondents to address five critical compliance criteria: execution of a distinct element of work, actual performance or management of work obligations, performance of work normal to the business function, responsibility for negotiating pricing and quality of products and materials while making payment, and limitation of subcontracting to levels consistent with normal industry practices. A separate form must be submitted for each certified small, micro, or disabled veteran business entity involved in the proposal. The document includes signature lines for both prime bidders and subcontractors, with fields to designate business type and document the date of submission.
